Skills and responsories required for the position include but are not limited to: - Ensuring that services are provided in a timely manner, informing families of their rights and procedural safeguards, coordinating funding sources, and assisting in the development of transition plans. - Process referrals, schedule initial meetings with families, and conduct Screenings using standardized protocol and tools approved by NYSDOH and BEI. - Coordinate, attend, and schedule assessments; IFSP (quarterly, 6 months, and annual), IEP, eligibility determination meetings, and transition planning; notify all interested parties of meeting dates and times. - Maintain extensive documentation to comply with state regulations and assist eligible children and their families in gaining access to Early Intervention services and other services identified in the Individualized Family Service Plan (IFSP). - Provide service coordination to infants and toddlers with developmental delays and their families through a comprehensive program of integrated services. Service Coordinators assist families in linking with community resources while offering support and guidance in child development.
