Summary Description: The Medical Director is a member of the leadership team at Greater Mental Health of NY (GMH). They will have primary responsibility for the supervision of medical and nursing staff that work as part of a multidisciplinary team in the Certified Community Behavioral Health Clinic (CCBHC) and Article 31 and 32 outpatient clinics which are licensed by the NYS Office of Mental Health (OMH) and/or Addiction Services and Supports (OASAS) which is part of GMH’s Comprehensive Care Center. This includes services to adults, children, and adolescents.
Key Responsibilities: 
<ol>
<li>Responsible for clinical support to those who provide direct care, including psychiatrists and psychiatric nurse practitioners, both in-person and remotely.</li>
<li>Support recovery by providing and promoting services that reflect our deeply held values that include the recognition of individual rights of self-determination, choice, shared decision-making and collaboration. </li>
<li>Utilize and promote evidence-based practices, always embracing a recovery-oriented, trauma-informed and person-centered approach. Confidentially, speak with clients if they are having problems with prescribers, medication, etc.</li>
<li>Supervise medication management in practices including medication assisted treatment (MAT) and psychiatric treatment. Keep current on practices and medications. Prescribe and/or approve any medications that are given to clients within the facility. This process may be done through other prescribers, but the Medical Director ensures that standards and philosophy are consistent and appropriate. If needed, administer medication – intramuscular injections according to Rx orders as documented in EHR.</li>
<li>Provide support, education and counseling to family members/significant others of clients to involve them in the treatment/recovery process. Provide public health education, including prevention and harm reduction.</li>
<li>Participate in the hiring process of medical staff and evaluate their clinical expertise.</li>
<li>Provide leadership in establishing treatment and standards of care within the agency.</li>
<li>Oversee the development and revision of medical policies, procedures and ongoing training in accordance with applicable laws and regulatory authorities.</li>
<li>Document client progress to maintain a permanent record of client activity.</li>
<li>Perform other duties as clinically necessary.</li>

Requirements:
<ul>
<li>Licensed psychiatrist currently registered by the NYS Education Department. </li>
<li>Experience in mental health and substance use disorder services required. </li>
<li>Must have board certification in addiction medicine from a certifying entity appropriate to their primary or specialty board certification and have at least 1 year experience in substance use disorder services.</li>
<li>Must meet the following characteristics:

<li>Waiver to prescribe buprenorphine (or obtain waiver to do so within 4-months of hire) and or one or more of the following: </li>
<li>Certification by the American Society of Addiction Medicine (ASAM). </li>
<li>Boarded by the American Board of Addiction Medicine; and/or </li>
<li>Board certification in Psychiatry with added qualification of certification in Addiction Psychiatry.</li>

<li>Hold a Federal DATA 2000 Waiver to prescribe buprenorphine. </li>
<li>Must comply with the standards and requirements of all applicable federal, state, local and other laws, rules, and regulations governing medical services. </li>
<li>Current Controlled Substance Registration issued by the Drug Enforcement Administration (DEA). </li>
<li>Demonstrate understanding of 42CFR, HIPAA Privacy Standards and related agency policies.</li>
<li>Must be reliable and able to work independently and understand the importance of maintaining confidentiality. </li>
<li>Must be cleared and maintain clearance by applicable regulatory clearances as required. </li>

This role is a transitional, entry-level position designed to train the trainee on the necessary skills required to become a Certified Nursing Assistant (C.N.A). Once classes have been successfully completed, employee may begin working as a nursing aide until certification is obtained.

The family therapist will do whatever it takes, via close collaboration with all involved, to address the needs of adolescents returning from residential placement for mental health, trauma, and in some cases, substance abuse. You will empower families to address challenging and/or problematic behavior and to help youth make life-transforming changes. Duties &amp; Responsibilities 
<ul>
<li>Provide direct clinical treatment using the MST treatment model and principles. Some principles include leveraging strengths and focusing on the positive, understanding sequences of behavior, and increasing mature behavior. </li>
<li>Conduct a thorough assessment of the client and family: gather information on behaviors of concern and strengths in the family and their ecology to inform conceptualization of the problem behaviors and interactions within the family’s ecological context. </li>
<li>Comfort working with a diverse community of clients</li>
<li>Knowledge of the types of families in the community</li>
<li>Continuously work to engage the primary caregiver, family members, supports, and community agency staff (school, outpatient mental health providers, residential case planners and line staff, child welfare, community stakeholders) in change-oriented treatment. </li>
<li>Dedicate time to weekly case planning and evaluation of case progress, with ongoing support from your supervisor and team members.  </li>
<li>Receive regular training, professional development, supervision, and consultation activities designed to help you acquire extensive clinical skills within the MST treatment model. </li>
<li>Assure, along with fellow clinicians, that clients have access to 24 hours/day, 7 days/week support as needed. </li>

Minimum Starting Salary: 62,000 -&nbsp; Qualifications 
<ul>
<li>Master's degree in clinical or counseling Psychology, Social Work, or a related subject area required.&nbsp;Eligibility for Professional Licensure in a behavioral health field is required. New master’s degree candidates are welcome with a limited permit.</li>
<li>Spanish Language fluency required</li>
<li>Willingness to travel throughout New York City required. Driver’s License preferred.</li>

SUMMARY:
As a Licensed Practical Nurse, you will provide professional patient care to long-term and post-acute care residents. You will direct and monitor the care provided by CNAs and consistently ensure proper documentation and recording is taking place. Your ability to lead is crucial in both the success of the CNAs and the care of the patients.
Attached to Clifton Springs Hospital, Clifton Springs Nursing Home offers a uniquely holistic approach to long-term care. Skilled nursing care is located adjacent to Spa Springs, where patients can access massage therapy, acupuncture and similar treatments. Clifton Springs Nursing Home is a 108-bed facility.
RESPONSIBILITIES:
· Responsible for timely, accurate medication administration of assigned team and concurrent required documentation.
· Performs treatments and documents progress of same as per physician orders and facility protocol.
· Assures concise, pertinent and complete resident care documentation using computerized medical record process. Assures the initiation of appropriate care pathway tools.
· Responsible for receiving/giving relevant reports, both written and verbal from/to appropriate staff as per departmental protocols.
· Assures proper monitoring of residents and relevant condition status are reported to charge nurse/supervisor and/or physicians in a timely manner.
· Completes pre-reviews of medication administration records as per facility protocols.
· Completes required MDS documentation and all related observations in a timely manner.
· Supports CNA team through assisting in direct ADL care of residents.
REQUIRED QUALIFICATIONS:
· Graduate from an accredited School of Practical Nursing.
· Current licensure in State of New York.
