Purpose of the Position
The Clinical Director is responsible for the programmatic service delivery in partnership with the Program Director, with emphasis on Behavioral Health Services. Responsibilities include implementation of new program design to bring intensive clinical services into program plan for clients. Other responsibilities include evaluation, reporting, supervision of clinical and social service staff. The Clinical Director uses Evidenced Based Practice (EBP), treatment plans, case management, crisis intervention, and discharge plans to assist the Social Services team with person centered planning for all Win clients.
Responsibilities:
• Supervise the program’s multi-disciplinary team, which will include Social Service Supervisors, Case Managers, and Client Care Coordinators.
• Provide supervision to Social Service Supervisors to troubleshoot client issues, and enhance direct reports clinical case management skills in accordance with program policies, procedures, and protocols.
• Provide clinical supervision to Client Care Coordinators to ensure supportive counseling is provided to clients.
• Provide coaching, training, and staff development to the Social Service team by incorporating social work values into program services, embracing a clinical lens.
• Provide management oversight to the supportive housing process.
• Train and support staff in the use of Evidence Based Practices (EBP), promote Trauma Informed Care (TIC) and Motivational Interviewing (MI), Critical Time Intervention (CTI) to exceed program goals and help clients achieve permanent housing.
• Conduct audits of the Uniform Client Chart and supporting documentation to ensure compliance with Office of Temporary and Disability Assistance (OTDA), Department of Homeless Services (DHS), contractual requirements and Win standards.
• Establish linkages with appropriate community agencies for services required by shelter residents, to include medical, mental health, substance abuse, child welfare and other stakeholders working with Win clients.
• Conduct community outreach and liaison with ancillary programs to maximize onsite care-coordination and referral process; establish long-lasting partnerships.
• Monitor incident follow-ups, ensuring optimal clinical lens during crisis intervention; provide direct client care and assessment to identified clients.
• Identify and consult in coordination of care for high-risk clients in collaboration with Program Director to make clinical decisions regarding client care and safety.
• Monitor and engage all long-term stayers within the facility by conducting case conferences with families, as well as monthly meetings with interdisciplinary staff as it relates to long term stayers.
• Liaison with ACS and ensure to attend all meetings related to ACS/Preventive interactions and provide information as a Mandated Reporter to the Administration for Children’s Services (ACS) as needed.
• Support Program Director in preparation of reports in accordance with Win and regulatory agencies.
• Perform other related duties as needed.
Essential Functions:

• Must be able to escort clients to appointments as needed.
• Must be able to work evenings, at least weekend on call monthly, and some holidays.
Qualifications:

<li>Complete comprehensive, recovery-oriented initial service plans and six-month service plan reviews, and modify the plan based on feedback from recipients and relevant others.</li>
<li>Involve social supports in the recipient’s treatment alongside the ACT team.</li>
<li>Complete progress notes within 24 hours of service delivery and ensure they are available for review in the morning organizational meeting.</li>
<li>Provide treatment services including engagement, problem-solving, wellness self-management, medication support, family support and treatment, mental health care, relapse prevention, harm reduction, and substance use counseling based on motivational interviewing and cognitive behavioral approaches.</li>
<li>Work effectively with community providers to ensure ACT recipients receive all entitled services.</li>
<li>Complete the expected minimum monthly service contacts each month.</li>
<li>Provide weekly reports to AOT staff for recipients under AOT orders.</li>
<li>Provide individual employment and vocational counseling to recipients pursuing vocational goals, and act as an advocate and liaison to help them access adult education, vocational training, and competitive employment.</li>

Job Description
All phlebotomists shall perform their duties in alignment with the vision and values of United WestLabs, Inc. The person doing this job is responsible for a variety of tasks that lead to a high level of quality of customer care and satisfaction in a cost effective manner. Under the direction of the Operations Supervisor, this position shall perform phlebotomy on patients. 
Duties and Responsibilities:
1. Perform phlebotomy professionally, competently and with compassion for the patient.
2. Maintain a safe and clean environment for all staff members and patients, including without limitation, collecting and disposing of specimens and other contaminated material in accordance with established policies and procedures and with adherence to the appropriate standards of infection control and body fluid precautions and protective equipment when necessary.
3. Ensure that requisitions are completed properly, and the patient demographic, diagnosis, insurance and physician information are included on or attached to the requisitions. Diagnostic information shall be in a narrative or ICD-9 code format and valid only if legible.
4. If a patient is a Medicare patient, you must review the National Coverage Decisions (“NCD”) and Local Medical review Policies (“LMRP”) for the tests ordered on the requisition form to determine if an Advance Beneficiary Notice (“ABN”) must be completed. If the form is needed, correctly complete the form with the patient.
5. Ensure that the Medicare Secondary Form (“MSP”) is completed when necessary.
6. Accurately collect all specimens necessary to fulfill the clients’ lab orders.
7. Provide accurate communication to clients and their staff regarding the proper ordering of lab tests.
8. Ensure that draw station where you work is kept clean and adequately stocked with supplies every day.
9. On a daily basis, inform your Operations Supervisor of your job performance issues, concerns and schedule.
10. If you work for UWL and are located within a physician’s office, you are not permitted to do any work for the physician and/or the physician’s medical practice, your sole responsibility is to collect specimens to be sent to a UWL client’s laboratory. 
Additional Responsibilities:
 1. Perform the duties of a courier if called upon. 
2. Deliver supplies when necessary.
3. Obtain any additional information if called upon.
4. Contact Medicare patients to fulfill compliance requirements in regard to MSP forms.
5. Any other reasonable task you may be asked to perform by the Operations Supervisor and which is not included herein.

We are currently seeking a Phlebotomist to work at our 400 Broadway location.
 
Position Summary: 
Perform venipunctures and finger sticks, process medical laboratory tests and document results for the diagnosis, treatment and prevention of disease. Follow all CLIA and COLA requirements and regulations. Serve as a back-up for clinical team members as needed. 
 
The major duties of the position include, but are not limited to:
<ul>
<li>Direct Patient Care - Obtain blood specimens from patients per laboratory orders by performing venipunctures and finger sticks, process medical laboratory tests and documents results.</li>
<li>Serve as a back-up for clinical team members as needed by controlling patient flow, performing EKG tests, obtaining blood pressure measurements, documenting vital signs in EMR and administering injectables.</li>
<li>Administration - Enter patient demographics, tests performed and diagnoses correlating to tests performed in the EMR.</li>
<li>Complete lab requisitions forms and produce appropriate labels for forms and specimen tubes.</li>
<li>Complete appropriate laboratory forms documenting tests performed and providing diagnoses correlating to tests performed.</li>
<li>Maintenance of Clinic Equipment and Supplies - Order and maintain adequate laboratory supplies.</li>
<li>Clean and maintain laboratory including equipment.</li>
<li>Compliance - Maintain records and documentation for proficiency testing and to meet COLA and CLIA requirements and regulations.</li>
</ul>
 
Qualifications: 
<ul>
<li>High school diploma or GED required.</li>
<li>Completion of accredited Medical Assistant Program, Associate degree preferred.</li>
<li>One (1) year full time Medical Assistant work experience preferred.</li>
<li>Possession of a current basic cardiopulmonary resuscitation (BLS CPR) certificate. </li>
<li>Ability to work both independently and in a team. </li>
<li>Experience working with people living with HIV/AIDS preferred.</li>
<li>Ability to speak Spanish or one of the Asisan languages is strongly preferred.</li>
