General Purpose:
Manage a caseload by providing individual assistance and guidance to individuals with disabilities with the mission of assisting them with obtaining, training and maintaining employment. The Peer Navigator will work as a fully integrated team member providing peer recovery support to individuals in outpatient, residential, and community settings. The Peer Navigator will also serve as a role model, mentor, advocate, and motivator for individuals in recovery, their family members, friends, and allies to support long-term recovery. 
 
Responsibilities
Direct Care of Recovery Services (70%)
<ul>
<li>Provide orientation to individuals regarding the process of their treatment when they begin accessing services.</li>
<li>Work with individuals and their case manager/social worker to create and execute a Recovery Wellness Plan (coach clients to transition from a professionally directed service plan to a self-directed Recovery plan) and evaluate progress.</li>
<li>Assist recovering persons to identify their goals, strengths, and personal interests regarding recovery and develop long and short-term strategies for achieving these goals.</li>
<li>Advice, guide, and train individuals through their recovery process as needed. </li>
<li>Escort individuals to appointments with agencies, self-help groups, and other community resources as needed.</li>
<li>Actively identify and support linkages to community resources that support the recovering person’s goals and interests. </li>
<li>Promote self-advocacy by assisting individuals to have their voices fully heard; their needs, goals, and objectives established as a focal point of rehabilitation and clinical services.</li>
<li>Participate with clinical staff in crisis intervention.</li>
<li>Report problematic issues, obstacles, and accomplishments experienced by individuals working toward their respective recovery goals to their case manager/social worker.</li>

The Certified Recovery Peer Advocate provides patients advocacy, outreach, education, support services. Skills and competencies required for the position include communication, cultural competence, training professional experiences, and education. The activities are tailored to meet the unique needs of the communities. This position includes no managerial or supervisory responsibilities.
KEY ESSENTIAL FUNCTIONS: 
<ul>
<li>Provide support for adults diagnosed with substance use disorders.</li>
<li>Working as a member of multidisciplinary treatment team that practices client centered and holistic care; accompany clients to appointments and complete warm- hand off referrals, including health homes services.</li>
<li>Communicating effectively with clients and collateral contacts and conduct outreach for care coordination.</li>
<li>Provide direct coaching, education, and advocacy in linking, engaging and retaining clients in services identified in the care of plan.</li>
<li>Escort clients to appointments and provide and gather critical information, both in the field and in the office, with the goal of health and wellness promotion and a reduction in preventable negative health or social events.</li>
<li>Participate in staff and organizational meetings as requested for care coordination and development of policies and procedures.</li>
<li>Maintain compliance with all organizational policies and procedures.</li>
<li> Participates in integrated meetings and indicators as per grant requirements</li>
<li>Conduct outreach and engagement such as interim visits with individuals after a discharge from a hospital, rehab, or inpatient stay to facilitate community tenure and increased readiness while waiting for the first post- discharge visit with a community substance use disorder provider.</li>
<li>Assist in the selection and utilization of self-directed recovery tools such as relapse prevention planning as part of assisting clients with self-help tools.</li>
<li>Connect individuals to self-help groups in the community.</li>
<li>Provide recovery education and counseling to individuals and their family members.</li>
<li>Provide transitional supports- bridging from an institution (jail, hospital, and rehab) to an individual’s home.</li>
<li>Provide pre-crisis and crisis support services.</li>
<li>Participate in staff and organizational meetings as requested for care coordination.</li>
<li>Maintain compliance with all organizational policies and procedures.</li>
<li>Participate as member of the EBPs groups’ curriculum development process.</li>
<li>Complete wellness, health promotions activities </li>
<li>Follow-up with the individual and the individual’s family/support network to confirm linkage to Care Coordination, outpatient treatment or other community services.</li>
<li>Perform additional relevant duties as requested by supervisor/management.</li>
<li>Complete office duties as needed/required </li>

REQUIREMENTS:

<li>Bi-lingual (Spanish/English); strongly preferred.</li>
<li>NYS OASAS Certified Recovery Peer Advocate certification is required. Provisional certification will be accepted within 12 months to obtain full certification. </li>

The Peer Mentor will achieve specific target recruitment goals each quarter for the Brooklyn and Harlem location. 

Job Responsibilities:

<li>The Peer Mentor will collaborate with the directors to develop and implement a recruitment strategy to identify fathers. </li>
<li>The Peer Mentor will support the Fatherhood Initiative to deliver eight required activities: 1. Orientation, 2. Parenting Skills Training, 3. Supervised Visitation Coaching, 4. Case Management, 5. Community Projects, 6. Support Groups, 7. Family Events, and 8. Workshops.</li>
<li>The Peer Mentor will support The Fatherhood Initiative serve and customize services for fathers with varying levels of child visitation, coparent engagement, court and agency orders, skills, and needs. Fatherhood will offer four service tier options to participants. The Fatherhood Initiative will implement parenting skills training to participants with two different curricula: ProFatherhing15 and Supporting Father Involvement (SFI).</li>
<li>Provide individual, family and group counseling, provide direct services/case planning to clients including mediation/conflict resolution training to fathers to facilitate effective co-parenting and visitation. When necessary, Case Managers will assist with supervised or monitored visitation opportunities for fathers.</li>
