POSITION SUMMARY:
The Care Coordination Program focuses on prompt linkage to HIV care, ongoing retention in care, optimal treatment adherence, viral load suppression and reduced HIV-related mortality. The programs in this department work with people living with HIV/AIDS to improve overall health and establish self-management through case management, peer navigation, and health education services.
 
The Supportive Counselors will play a dynamic role in enhancing these services by providing direct support and counseling services to clients by conducting assessments, developing individualized care plans, and providing ongoing support.
 
DUTIES &amp; RESPONSIBILITIES:

<li>Conduct comprehensive assessments and intakes to evaluate for client eligibility and goals</li>
<li>Develop individualized care plans to help clients stay engaged in their own care</li>
<li>Provide one-on-one supportive counseling sessions</li>
