THE OPPORTUNITY
Brooklyn Justice Initiatives (BJI), the largest operating project of the Center, seeks to re-engineer the experience of criminal court in Brooklyn, New York, by providing judges and attorneys meaningful alternatives to bail, fines, and jail sentences. Operating out of Kings County Criminal Court and community-based offices, BJI is a team of social service providers, court-based resource coordinators, mental health practitioners, compliance specialists, and others who seek to improve the quality of justice. Supervised Release offers an alternative to jail by providing pretrial supervision, case management, and voluntary social services to people charged with misdemeanor and felony offenses, and in doing so, uses an arrest as a window of opportunity to change the direction of a participant’s life, avoiding the harmful effects of incarceration. Program participants are monitored to ensure their appearance at court dates and mandatory programming, and receive referrals to services like job training, drug treatment, and mental health counseling.
BJI seeks Night Clinical Intake Specialists for the borough’s Supervised Release Program (SRP). Reporting to the Night Clinical Intake Coordinator, Clinical Intake Specialists will leverage social work or mental health training and direct practice experience to facilitate participants’ successful transition into SRP. Clinical Intake Specialists will conduct a comprehensive needs assessment; connect participants with an appropriate case manager, focusing on participant needs and individual case manager skills and strengths; generate mandatory programming recommendations, and refer candidates to services with community-based service providers. Additionally, this position will provide de-escalation and emergency intervention within the courthouse, as needed.&nbsp;
Night Clinical Intake Specialists are responsible for effective cross-team communication and collaboration to prioritize comprehensive and aligned service delivery; proactively identifying successes, challenges, and opportunities to strengthen programming; create opportunities for meaningful collaboration, and maximize the use of internal and external resources and expertise.&nbsp;

We are seeking a full time Medical Receptionist for our busy White Plains Ophthalmology practice.
This position requires multi-tasking effortlessly, prioritizing efficiently, handling patient requests and handling all administrative tasks with a positive attitude (to include patient check-in, check-out, receiving payments, and answering telephones).

Responsibilities:
<ul>
<li>Acquaint yourself with all new and established patients, providing plan/setting expectations for visit and guide patients to examination rooms</li>
<li>Assist the physician with documentation of the patients’ medical record</li>
<li>Accompanies the physician into the exam room to transcribe the history of the examination as given by the patient and the physician</li>
<li>Under the direction of the physician, transcribes the impression and plan, results of tests, prescriptions, and orders</li>
<li>Documents and aids in any procedural set that may be performed by the physician</li>
<li>Document all history and testing information accurately and consistently</li>
<li>Summarize all exam details from physician into patients’ electronic chart, including but not limited to: proper charting of examination, additions to chief complaint, physician assessment and recommendations for treatment</li>
<li>Other duties as assigned or directed by your supervisor</li>
</ul>
Required Qualifications:
<ul>
<li>Experience as a Medical Scribe is required. Ophthalmology and retina experience preferred. &nbsp;&nbsp;</li>
<li>You should be able to communicate professionally, be comfortable working in a medical environment, be highly organized, have a strong customer service background, possess excellent computer and organizational skills, and champion a welcoming/friendly demeanor</li>
<li>Knowledge of Electronic Medical Records (EMR) and Medical Terminology</li>
<li>Working knowledge of eye anatomy, retina disease, and ocular medications</li>
<li>Basic to advanced computer skills required</li>
<li>Strong written and verbal communication</li>
</ul>
Preferred Qualifications:

CAMBA’S Office of Mental Health FUNDED Highbridge Overlook Supportive Housing Program will target and provide 40 units of NY/ NY III congregate care housing to chronically homeless (SPMI) population A: 40 units for individuals with a serious mental health illness who either reside in a shelter operated by NYC Department of Homeless Services or who are designated as street homeless. Units are in the Bronx.
Position: Case Manager

What The Case Manager Does:
<ul>
<li>Maintain professional relationships with clients and client confidentiality.</li>
<li>Practice Universal Precautions/Standard Protocol &amp; Procedures.</li>
<li>Comply with any and all Federal, State, City and CAMBA security and privacy polices intended to protect the security and privacy of individually identifiable health information.</li>
<li>Review all documentation establishing tenants’ eligibility for program and make file copies.</li>
<li>Create and maintain tenant files.</li>
<li>Conduct initial intake and assessment of tenants and tenants’ families’ needs and periodic re-assessments.</li>
<li>In collaboration with tenants, prepare initial and periodic revisions of independent living plans including short-term and long-term tenant goals.</li>
<li>Assist tenants in attaining their goals by identifying and locating community resources for tenants and by making referrals to appropriate services both within and outside CAMBA.</li>
<li>Work with tenants to break through barriers to tenant goals and to assist tenants in advocating for themselves and in moving toward self-sufficiency.</li>
<li>Recommend and implement strategies to persuade tenants to participate more fully in this process.</li>
<li>Monitor tenants’ progress toward their goals via regularly scheduled telephone contact and/or face-to-face visits, and document via progress notes.</li>
<li>Participate in case conferences.</li>
<li>Follow-up with tenants and with referral organizations regarding tenant contact and progress with referral organization.</li>
<li>Provide all required information for weekly/monthly/quarterly/annual reports.</li>
<li>May recommend closing of cases in which tenants have: (a) achieved primary goals and have maintained stability for a period of months; or, (b) have not demonstrated a willingness to participate in the process (lost-to-service); or, (c) have become ineligible for services (e.g. moved out of building).</li>
<li>May act as tenant liaison/tenant advocate with outside organizations regarding such matters as education, healthcare, housing, legal issues, etc.</li>
<li>May schedule appointments for tenant with referral organizations.</li>
<li>May escort tenants to appointments (educational, medical, social service, etc.)</li>
<li>May assist tenants in completing applications for benefits and entitlements, and may process applications on tenants’ behalf.</li>
<li>May follow-up with tenants for a period of time after successful completion of their primary goals to assure tenant stability.</li>
<li>May evaluate actual living conditions of tenants through home visits.</li>
<li>May prepare marketing materials for the program.</li>
<li>May prescreen tenants over the telephone for eligibility and may schedule intake appointments.</li>
<li>May input tenant data and tenant progress information into automated database.</li>
</ul>
Education, Experiences, Licenses, and/or Certifications Required:
<ul>
<li>Bachelor's degree (e.g., B.A., B.S.W.) or</li>
<li>Four years of relevant experience, or</li>
<li>Associate's and two years equivalent experience.</li>
</ul>

Compensation: $45,000-$50,000 annually When salary ranges are listed, the range would represent the low and high end for the applicable position &amp; program. The salary offered would be based on various factors unique to each program and candidate.&nbsp;This includes but is not limited to experience, education, budget and/or program size, internal equity, skills and other factors that may be required for the position and organization.
