The Clinician plays an essential role in providing in depth assessment and ongoing mental health and emotional support to Unaccompanied Children (UC). This role involves addressing the child’s safety and well-being by coordinating services, conducting in-depth assessments, and ensuring that minors are connected to necessary mental health resources. The Clinician is expected to conduct a more comprehensive and intensive assessment of the child to develop and execute tailored case plans to ensure long-term stability. This position requires interaction with community services, educational agencies, and other stakeholders.
Qualifications

<li>Clinical responsibilities include all necessary tasks as required by the Federal Office of Refugee Resettlement </li>
<li>Ensure all ORR, OCFS mandates and requirements are met. </li>
<li>Conducts comprehensive assessments using standardized measures as per standards set forth by Office of Refugee Resettlement (ORR). </li>
<li>Provides individual and group therapy. Makes referrals for additional mental health services as needed. </li>
<li>Provides crisis management and ongoing therapeutic support. </li>
<li>Create Behavioral Plans, Individual Crisis Management Plans and Safety Plans as needed to provide ongoing support to UC and foster parents. </li>

As a Clinical Care Specialist at Horizon, you will…

<li>Provide various forms of care coordination by relaying medication statuses with patient/counselor and following up on completion of releases, paperwork, and status update reports, and/or applications.</li>
<li>Work closely with multidisciplinary clinical teams</li>
<li>Conduct toxicology tests with patients</li>
<li>Respond to calls and walk-ins presenting at the clinic in crisis situations.</li>
<li>Review and flag patient records to ensure required updates are prepped and/or made timely.</li>
<li>Successfully complete internal training and credentialing process to perform approved clinical services such as educational and process group activities.</li>
<li>Relate to mentally ill or chemically dependent individuals and their families of diverse ethnic, cultural, and socioeconomic backgrounds, as well as have the ability to interact with them in a positive, supportive, and cooperative manner.</li>
<li>Experience working in a computerized office environment with word processing, database and spreadsheet skills sufficient to prepare correspondence, reports, and forms, including the willingness to learn new software applications.</li>
<li>Complete records requests (disability requests/attorney requests/patient request, etc). This may vary by clinic.</li>
<li>Monitors clinical and non-clinical data via internal Electronic Health Record (EHR), internal dashboards and reports, as well as and regional informational health systems. </li>
<li>Provide support and triage to patients relative to importance of following up with behavioral health and medical providers, as appropriate.</li>
<li>Help offer support to patients being seen for medication only services</li>
<li>Support the clinical treatment team including providers and clinicians including follow up and outreach for engagement purposes.</li>
<li>Participates in data collection with active patients</li>
<li>Provide support to patients relative to importance of following up with behavioral health and medical providers, as appropriate.</li>

Compensation
Horizon is consistently evaluating our compensation and benefits packages to ensure we remain competitive. The pay range for this position is $19.50 - $21.00. Starting pay may be impacted based on experience and education. Compensation adjustments are evaluated annually.

Job Responsibilities:
As a Licensed Clinician&nbsp;with Family Services, you’ll have the opportunity to work with individuals in the following ways.

<li>Provide clients with individual and group therapy.</li>
<li>Assist with the pre-intake, intake, and screening of new clients into the system and providing information on their psycho-social history. </li>
<li>Complete a differential diagnosis for clients and refer them to the appropriate program(s) and/or case manager/licensed prescriber for services not under this position’s jurisdiction.</li>
<li>Create a treatment plan for clients, modify on an ongoing basis in reflection of their progress in therapy and prepare them for the completion of their treatment.</li>
<li>Complete administrative tasks including, but not limited to, maintaining case records, documenting all clients’ contacts, and meeting deadlines, responding to outside agency queries regarding client information.</li>
<li>Develop and implement follow-up studies to determine program effectiveness.</li>
<li>Coordinate with appropriate parties on client’s treatment (i.e., schools, Social Services, probation department). </li>
<li>Provide clients with information and educational community activities to combat substance use and mental health issues.</li>
<li>Act as a client advocate and liaison with community resources.</li>
<li>Provide training to colleagues on issues related to mental health treatment.</li>
