The duties and responsibilities of the Residence Program Specialist will include but are not limited to the following:

Certified Occupational Therapy Assistant, COTA, Position Summary:
EPIC Rehabilitation and Nursing at White Plains is a BEAUTIFUL *5 Star* 160-bed NEW skilled nursing &amp; rehabilitation center opened in 2020 in the heart of White Plains. <li>Certified Occupational Therapy Assistant, COTA will establish and perform treatment programs for assigned residents.</li>
<li>Certified Occupational Therapy Assistant, COTA will upgrade goods as appropriate and physician order recommendations.</li>
<li>Certified Occupational Therapy Assistant, COTA will enter billing information in an accurate and timely manner.</li>
<li>Certified Occupational Therapy Assistant, COTA will report resident progress in meetings, i.e. Medicare A Meetings. Consult with other members of rehabilitation team to coordinate occupational therapy with other therapeutic activities.</li>
<li>Provide residents with equipment as appropriate &amp; inform Social Services of equipment needs at discharge. Requisitions supplies and equipment.</li>
<li>Design and construct special equipment for individuals and suggest adaptation of individual’s living environment.</li>
<li>Lay out materials for individual’s use and return equipment to appropriate place.</li>
<li>Conduct training programs for other workers/families in occupational therapy techniques and objectives for resident care.</li>
<li>Chart notes in an informative and descriptive manner that reflects the therapy provided to the residents as well as the resident’s response to the therapy.</li>
<li>Perform routine documentation as required and in accordance with our established charting and documentation policies and procedures. Sign and date all entries made in resident’s medical record.</li>
<li>Follow all facility policies and procedures.</li>
<li>Perform other related duties as required and assigned.</li>
</ul>
Requirements:
<ul>
<li>The Certified Occupational Therapy Assistant is trained and licensed, meeting the qualifications set forth by the NYS Board of Education and the NYS Rules and Regulation requirements.</li>
<li>Graduate of an approved Certified Occupational Therapy Assistant Program.</li>
<li>Training certification by the AOTCB and current licensure in good standing with the NYS Board of Education.</li>
<li>One (1) year experience in the skilled nursing facility setting, preferred.</li>
<li>Experience with MDS, PRI and Comprehensive Care Planning. Entry-level knowledge and professional competency of the practice of occupational therapy as set forth by the NYS Board of Education and the AOTA.</li>
<li>Candidate will have the physical ability to perform job related duties, which may require lifting, bending, stooping, stretching, pulling and pushing up to 50lbs or more as needed.</li>
<li>Work well under pressure &amp; willing to work harmoniously with all staff</li>
<li>Sincere desire to work with the elderly, chronically ill, and handicapped</li>
<li>Excellent customer service skills</li>
<li>Must be able to communicate well verbally with residents, families and staff</li>
<li>Treat residents and fellow staff with respect, dignity, and compassion</li>

Job Description 
We are looking to hire Physical Therapists to conduct home evaluations for Healthfirst members around New York City. The Physical Therapist will be responsible for completing evaluations and treatment status updates. The assigned Physical Therapist will be responsible for completing a home safety checklist to prevent falls, in addition to determining current states of function. 
Responsibilities of the Physical Therapist
<ul>
<li>Provides evaluation and treatments in accordance with professional standards of practice. </li>
<li>Observe precautions related to infection control and resident/patient privacy and dignity. </li>
<li>Completes all documentation in a timely and accurate manner, in accordance with departmental, regulatory, and payor standards.</li>
<li>Assists in maintaining the safe operation of all equipment for the safety of patients. </li>
<li>Accurately integrates treatment knowledge into the daily education of patients and caregivers </li>
</ul>
Requirements of the Physical Therapist
<ul>
<li>Registered Physical Therapist with current registration to practice in New York State.</li>
<li>Degree from an approved/accredited program/school for Physical Therapy</li>
<li>Professional pediatric care experience.</li>
<li>The successful candidate will also recommend treatment and appropriate therapeutic techniques specific to the age and condition of each patient. </li>
