JOB DESCRIPTION:
Graduate Nurses promote residents' health, wellness, and illness prevention through daily nursing and personal care endeavors. A Graduate Nurse works under the direction of a registered nurse and must be ready to handle a diverse range of tasks and responsibilities, including administering treatment or medications, performing clinical procedures, and documenting the clinical data of patients.
RESPONSIBILITIES:

<li>Responsible for the care and safety of all patients/residents on the nursing unit, ensuring that individual care plans are current, implemented and revised as necessary</li>
<li>Administers and documents medications, treatments and other nursing services to ensure that the medical record reflects progress and the current condition of the patient/resident</li>
<li>Ensures that the medical record reflects progress and the current condition of the resident at all times</li>
<li>Receives report from the off going team leader/nursing supervisor, makes rounds to verify the needs of the residents</li>
<li>Ensures that the designated representative is notified of any change in the patient/residents condition or plan of care</li>
<li>Ensures that the physician is informed on any change in the patient/residents condition, abnormal lab results, diagnostic test results, consultant recommendations, etc.</li>
<li>Reports pertinent information regarding patients/residents to the oncoming team leader/nursing supervisor</li>
<li>Possesses knowledge of the facility’s policies and procedures, abides by these policies, ensures that the staff is informed of and correctly carries out these policies</li>
<li>Maintains HIPAA privacy, security and confidentiality standards</li>
<li>Follows universal precautions and infection control techniques</li>

Requirements:

<li>High school diploma or equivalent preferred</li>
<li>A graduate nurse currently eligible for licensing in the State of New York, who possesses an appropriate level of education, training, and experience to fulfill all requirements/duties of the position</li>

SCHEDULE: D/E/N with rotating every other weekend
SUMMARY
The Licensed Practical Nurse is responsible for providing direct patient care, usually under the direction of a Registered Nurse, in accordance with established plans.
ELDERONE
An affiliate of Rochester Regional Health, a Program of All-Inclusive Care for the Elderly (PACE). Servicing Monroe, Ontario and Wayne counties, ElderONE is an innovative alternative to long-term nursing home placement.&nbsp;As the area’s only PACE program, ElderONE represents the future of senior health care. This long-term care program provides the entire continuum of medical care and support services to seniors 55 and older with chronic care needs, allowing them to maintain their independence in the comfort of their home without compromising their health or safety.
 RESPONSIBILITIES

Patient Care &amp; Service. Promote and restore patients' health by completing the nursing process; collaborate with physicians and multidisciplinary team members; perform various treatment procedures; provide physical, educational and emotional support to patients, friends and families; supervise assigned team members
</li>

Documentation. Ensure concise, pertinent and complete resident care documentation using computerized medical record process
</li>
<li>
Medication Administration &amp; Reporting. Administer medication and give/receive relevant reports, both written and verbal from/to appropriate staff as per departmental protocols

Required Licensure/Certification Skills: •Current licensure in the State of New York

BLS - Basic Life Support - American Heart Association (AHA), LPN - Licensed Practical Nurse - New York State Education Department (NYSED)
PHYSICAL REQUIREMENTS:
M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.
