POSITION DESCRIPTION
Upper Room AIDS Ministry (URAM), a Federally Qualified Health Center under Harlem United, provides high-quality primary, specialty, dental, and wellness care in the heart of Harlem. All URAM employees are expected to uphold best practices in risk management, quality assurance, and exceptional patient-centered service.
The Licensed Practical Nurse (LPN) plays a critical role in patient care, providing direct clinical support, assisting with diagnostic testing, vaccinations, and information gathering, while also managing key administrative tasks such as hospital discharge follow-ups, vaccine inventory, prior authorizations, and chronic disease monitoring. This role requires both clinical expertise and strong organizational skills to ensure seamless healthcare delivery.
Under the supervision of the Nursing Manager or Physician, the LPN collaborates within a multidisciplinary team to provide high-quality, patient-centered care. The position requires strict adherence to New York State Department of Health regulations and scope of practice guidelines. The ideal candidate is detail-oriented, efficient, and capable of managing a fast-paced workload with accuracy.
ESSENTIAL DUTIES AND RESPONSIBILITIES 
Primary Care Nursing Responsibilities
Clinical

<li>Facilitate efficient patient flow within the primary care clinic.</li>
<li>Administer injections as ordered by providers.</li>
<li>Ensure exam rooms are fully stocked and prepared for patient care.</li>
<li>Coordinate and prepare for Care Team Huddles to support team collaboration.</li>
<li>Maintain accurate and timely documentation in compliance with protocols.</li>
<li>Prepare equipment and aid physician during treatment, examination, and testing of patients</li>
<li>Verify chief complaint and take vital signs as requested.</li>
<li>Review the EHR active medication list with patients.</li>
<li> Conduct and document point-of-care tests based on protocol.</li>
<li>Perform testing, immunization, and other injections, as directed by providers and clinic protocols.</li>
<li>Perform phlebotomy as needed. </li>
<li>Step in to perform key Medical Office Assistant (MOA) responsibilities when needed to maintain seamless patient care.</li>

Administrative
<ol>
<li>Perform prior authorizations and DME requests.</li>
<li>Monitor and update patient monitoring databases (e.g hospital discharges, injectables, DOH reporting.)</li>
<li>Monitor and respond to clinical messages.</li>
<li>Under PCP’s direction, provide abnormal test results to patients and provide recommendations.</li>
<li>Assist patients in completing medical forms.</li>
<li>Track and record vaccines provided through the Vaccines for Children (VFC) program.</li>
<li>Monitor and document vaccine refrigerator temperature logs.</li>
<li>Assist in coordinating patient care within the clinic and with external providers.</li>
<li>Participate in collaborative activities to enhance patient outcomes.</li>
<li>Maintain all environment of care quality logs</li>
<li>Participate in scheduled clinic rounds to maintain the environment of care</li>
<li>Ensure that the quality of care and environment are acceptable.</li>
<li>Administer Directly Observed Therapy (DOT) as needed.</li>
<li>Provide patient education under the direction of the Nurse Manager or Provider.</li>
<li>Ensure adherence to Infection Control and quality of care standards.</li>
<li>Comply with all New York State Department of Health/AIDS Institute regulations.</li>
<li>Follow and uphold all nursing policies and procedures.</li>
<li>Attend nursing conferences, as approved, to stay updated on primary care best practices.</li>
<li>Support and engage in advocacy efforts addressing homelessness and HIV/AIDS.</li>
<li>Initiate, track and follow-up with prior authorizations.</li>
<li>Other duties as assigned by supervisor.</li>

EDUCATION AND CERTIFICATION REQUIREMENTS
<ol>
<li>Licensed Practical Nurse (LPN) with active licensure in the State of New York.</li>
<li>1-2 years of experience in an ambulatory care setting.</li>
<li>Current BLS certification for Healthcare Providers.</li>

Special Requirements

This position is a full-time, remote position that reports directly to the Clinical Manager of the SUD Program. 
The hours required for this position are Monday-Friday from 2pm-10pm EST (or equivalent hours in your respective time zone, ex: 11am-7pm PST)
As a full-time, Clinical SUD Navigator, you will play a pivotal role in supporting individuals with substance use disorders (SUDs). In this position, you will leverage your licensure and advanced skills to independently manage client care, provide expert guidance, and coordinate comprehensive support services. Your responsibilities include engaging with clients, triage and recommendations and collaborating with other SUD Care Navigators and interdisciplinary teams. A relevant license and extensive experience in mental health and substance abuse are required.
What you'll be doing
<ul>
<li>Conduct outbound calls and follow-ups with individuals interested in or referred for substance use disorder treatment, demonstrating advanced skills in engagement and relationship-building.</li>
<li>Engage with clients using advanced clinical techniques to assess their needs, provide comprehensive information about treatment options, and make informed recommendations for care.</li>
<li>Independently coordinate appointments, referrals, and follow-up care, ensuring a seamless and high-quality care experience for clients, including managing complex cases.</li>
<li>Apply advanced psycho-education, motivational interviewing, and evidence-based practices to support clients in setting and achieving their goals for reducing substance use and enhancing recovery.</li>
<li>Utilize in-depth knowledge of substance use disorders, ASAM levels of care, and various SUD treatments (e.g., contingency management, MAT, detox) to provide expert guidance and support.</li>
<li>Provide sophisticated in-the-moment support, crisis intervention, and safety planning for clients facing urgent or high-risk situations, utilizing advanced clinical judgment.</li>
<li>Maintain detailed and accurate records within an EHR and other systems, ensuring comprehensive documentation and tracking of client progress and interventions.</li>
<li>Identify and resolve complex issues that may disrupt care, employing advanced problem-solving skills and resourcefulness.</li>
<li>Collaborate effectively with multi-disciplinary teams, including care support, provider operations, and benefits/billing, to ensure integrated and efficient client care.</li>
<li>Actively participate in staff meetings, case consultations, and ongoing training to continuously enhance clinical skills and knowledge.</li>
<li>Adhere strictly to confidentiality, compliance, and ethical standards as outlined by organizational policies.</li>
<li>Provide support and coverage to other SUD Clinical Navigators, offering guidance and assistance as needed to ensure team cohesion and effectiveness.</li>

What we expect from you
<ul>
<li>Mental Health Licensure required: LCSW, LMFT, LPC with 2+ years of experience in substance use disorder (SUD) clinical care.</li>
<li>Proficiency in motivational interviewing, triage, ASAM criteria and various levels of care and best practices for SUD. Strong listening, written, and verbal communication skills.</li>
<li>Ability to work effectively with diverse populations and a commitment to embracing diversity and inclusion.</li>

The target base salary range for this position is$33.33 - $42.31 Hourly, and is part of a competitive total rewards package including stock options and benefits. Individual pay may vary from the target range and is determined by a number of factors including experience, location, internal pay equity, and other relevant business considerations. We review all employee pay and compensation programs annually using Radford Global Compensation Databaseat minimum to ensure competitive and fair pay.
