Extended Day Activity Instructors lead afterschool programming for children and teens at New York Edge after-school program sites. Education &amp; Experience:
<ul>
<li>Lived, experience and willingness to utilize personal experience to assist others in recovery.</li>
<li>High School Diploma</li>
<li>Six months to one year current or recent employment.</li>
<li>Six months to one year part time or full time experience in delivery of direct services to mental health consumer/survivors (volunteer or paid), preferred.</li>
<li>Three months first hand experience in self-help or peer directed services and/or peer support groups, preferred.</li>
<li>NYCPS-P Certification Preferred</li>

Other Qualifications

<li>Good oral and written communication skills.</li>
<li>Completion of Wellness Recovery Action Plan (WRAP) training preferred.</li>

Job Title: Peer Care Manager Reports To: CTI Team Leader Program: Critical Time Intervention (CTI) Location: Tarrytown NY/Mobile Hours per Week: 35 FLSA Status: Full Time,Non-exempt Salary Range: $24.18/hour - $28.57/hour
Summary Description: The Peer Care Manager will work as part of a multidisciplinary team in the Critical Time Intervention (CTI) program serving adults with co-occurring behavioral health and physical health needs who frequent ED/inpatient settings. The Peer Care Manager provides CTI services from a lived-experience and non-clinical perspective and carries a caseload. Peer Care managers develop the person-centered care plan, share knowledge of community resources, provide linkage to resources, engage individuals in skill building, offer crisis management, collaborate with other providers, assist individuals in navigating complex systems, and work with informal supports.
Key Responsibilities: 
<ul>
<li>Manage a caseload of clients consistent with industry standards, meet regulatory expectations/ guidelines and reflect our client population's needs. The exact number of clients may vary based on factors such as case complexity and the availability of support resources. The care manager is expected to maintain a balanced caseload for high-quality care and effective case management.</li>
<li>Provide comprehensive care management and care coordination, through face-to-face and telephonic services.</li>
<li>Complete assessments and approve applications for services and entitlements</li>
<li>With client’s input, create individualized plans of care </li>
<li>Assist in discharge planning during a client’s hospitalization and provide follow-up care as needed</li>
<li>Consult with team members and other specialists</li>

Requirements: 
<ul>
<li>Bachelor’s Degree in behavioral health field OR High School Diploma or equivalent plus at least six months experience of direct care experience with individuals with serious mental illness</li>
<li>New York State Peer Certification required, or provisional certification must be obtained within 6 months of employment.</li>
<li>Self-identify as a person who has first-hand experience with mental health, substance use, and/or related behavioral health challenges.</li>
<li>Be able to use this lived experience to support individuals in their recovery.</li>
<li>Good organizational, written, and engagement skills </li>
<li>Valid Driver's License and satisfactory driving record.</li>
<li>Maintain New York State Defensive Driving certification within 30 days of hire.</li>
<li> Basic computer knowledge: typing, communication tools, scheduling tools and web browsers.</li>
<li>Proficiency in Microsoft Office suite and database management systems. </li>
<li>Fluency in English (Reading, Writing, Speaking).</li>
<li>Must be and maintain clearance by applicable regulatory clearances as required.</li>

Special Considerations: 
<ul>
<li>This is a full-time, 35 hour per week position with a regular daytime schedule that will include an evening shift and rotating on-call responsibilities. </li>
<li>This position will require availability outside of the standard workday, and as such, must be ready and available at times to direct and support staff either by phone or in person if needed during the evenings, weekends, and holidays.</li>
