MAJOR FUNCTIONS:
The Community Recovery Coach is responsible for providing community-based recovery support to program participants. The Community Recovery Coach is responsible for a range of activities which assist participants to improve the quality of community-based recovery in order to remain in their homes and avoid institutional care. 
 
SPECIFIC DUTIES &amp; RESPONSIBILITIES:
<ul>
<li>Contributes to the development of the ethical framework for delivery of recovery services and conducts all recovery coaching activities within this framework.</li>
<li>Contributes to the development of resource book of long term supports and services.</li>
<li>Engages with assigned program participants within one week of enrollment for services. </li>
<li>Completes a Recovery Plan with each participant within 14 days of program enrollment.</li>
<li>Engages in problem solving activities with each participant to improve daily living skills. </li>
<li>Acts as a role model for effective health and recovery maintenance activities and promotes abstinence from substances of abuse. </li>
<li>Provides guidance, support and recovery coaching for participants using CCAR practices. </li>
<li>Engages in recovery support activities with participant.</li>
<li>Escorts participant to long term services and support appointments based on participant’s preference..</li>
<li>Conducts outreach activities as needed or directed.</li>
<li>Organizes and participates in community-based recovery supportive activities and events.</li>
<li>Delivers weekly individual recovery coaching sessions of at least 30 minutes duration to each assigned participant and documents in accordance with APG guidelines.</li>
<li>Attends monthly Recovery Coach Support group.</li>
<li>Facilitates weekly Peer to Peer recovery support groups.</li>
<li>Maintains weekly Recovery Coach Contact log.</li>
<li>Conducts educational presentations for providers of local recovery support services.</li>
<li>Conducts educational presentations to groups of prospective Recovery Coaches.</li>
<li>Conducts educational presentation to groups of prospective participants.</li>
<li>Eligible to serve on Consumer Advisory Board and project steering committee.</li>

REQUIREMENTS: EDUCATION/TRAINING AND EXPERIENCE KNOWLEDGE, SKILLS AND ABILITIES

<li>Must have Connecticut Community for Addiction Recovery (CCAR) certification, or to be obtained within six months of hire.</li>
<li>Ability to deliver culturally competent services to a diverse client population in an urban setting.</li>
<li>Familiarity with the projects ethical framework for recovery services and the Recovery Self-Assessment and Planning Guide.</li>
<li>A minimum ofsix months of community based recovery with at least six consecutive months of abstinence at the time of enrollment.</li>
<li>Good interpersonal skills, enthusiastic about recovery.</li>
<li>Must be able to work a flexible schedule.</li>
