Summary Description: The Peer Care Manager will work as part of a multidisciplinary team in the Critical Time Intervention (CTI) program serving adults with co-occurring behavioral health and physical health needs who frequent ED/inpatient settings. The Peer Care Manager provides CTI services from a lived-experience and non-clinical perspective and carries a caseload. Peer Care managers develop the person-centered care plan, share knowledge of community resources, provide linkage to resources, engage individuals in skill building, offer crisis management, collaborate with other providers, assist individuals in navigating complex systems, and work with informal supports.
Key Responsibilities:

Responsibilities: 
<ul>
<li>Work with students in the home and community setting to provide individualized lessons which target Individualized Education Plan (IEP) goals. </li>
<li>Develop creative and multi-sensory lessons based on IEP goals to engage and captivate learners with various abilities. </li>
<li>Collect meaningful data on IEP goals and report quarterly on child's progress with IEP goals. </li>
<li>Ongoing communication with lead supervisors and school team. </li>
<li>Maintain accurate attendance and submit electronic billing on a daily basis. </li>
<li>Knowledge of discrete trial instruction, natural environment teaching, and incidental teaching is a plus. </li>
<li>Attend professional development trainings to maintain best practice approach and continue personal development. </li>
<li>Maintain professionalism with parents, teachers, and other professionals. </li>
<li>Attend CSE meetings. </li>
