Overview: We are seeking a Rehabilitation Manager to support the clinical oversight and delivery of rehabilitation services within Managed Long-Term Care (MLTC), MAP, and ESNP programs. This role focuses on ensuring members receive appropriate care and equipment to support independence and quality of life at home.
Responsibilities:
<ul>
<li>Review rehabilitation documentation from external providers to ensure services are clinically appropriate.</li>
<li>Evaluate and process requests for Durable Medical Equipment (DME) for home use.</li>
<li>Provide education and guidance to nursing leadership regarding rehabilitation services and care planning.</li>
<li>Perform home and field assessments as needed (including evaluation for complex DME and home safety).</li>
<li>Maintain compliance with New York State Department of Health regulations.</li>
</ul>
Qualifications:
<ul>
<li>Master’s degree in a healthcare-related field.</li>
<li>Active New York State Occupational Therapist License (required).</li>
<li>2–4 years of experience as an Occupational Therapist.</li>
<li>Strong communication, organizational, and computer skills.</li>
<li>Ability to work effectively in a fast-paced, collaborative environment.</li>
<li>Willing to conduct home visits.</li>
<li>Must be able to travel within Bronx, Manhattan, and Westchester regions.</li>
