RN Unit Manager

Duties include:

Oversee daily operations of a unit serving up to 40 patients, across both short‑term rehabilitation and long‑term care programs.
Provide support with treatments and medication administration when needed to ensure continuity of high‑quality care.
Assist with new patient admissions, including assessments, documentation accuracy, and coordination with interdisciplinary teams.
Attend and actively participate in departmental and interdisciplinary meetings.
Perform daily audits of all newly admitted patient charts to ensure compliance and completeness.
Serve as a primary liaison among families, patients, physicians, and internal departments to promote clear communication and patient-centered care.
Investigate all incident reports, document findings, and complete required follow‑up actions.
Participate in staff management, including assigning duties and supporting team workflow to maintain quality standards.
Monitor, document, and report on patient conditions, communicating changes promptly to appropriate providers.
Contribute to annual staff evaluations, offering feedback and performance insights.
Take on‑call responsibilities on weekends and holidays as needed to support clinical operations.

Requirements:

NY RN License
Experience as as RN within a skilled nursing facility / long-term care facility is required
