Job Description

The Director of Patient Services (DPS) is a key leadership role responsible for overseeing the clinical operations and patient care services of a certified home health agency. The DPS ensures the delivery of high-quality, compliant, and compassionate care in accordance with federal, state, and agency standards. This role involves supervision of clinical staff, coordination of patient care, and collaboration with the interdisciplinary team to promote positive outcomes and patient satisfaction.

Key Responsibilities:

  • Provide clinical oversight and leadership for the planning, coordination, and delivery of patient care services.

  • Ensure compliance with all federal and state regulations (including Medicare Conditions of Participation), accreditation standards, and internal policies.

  • Supervise, support, and evaluate clinical staff, including nurses, therapists, and aides.

  • Oversee the development, implementation, and updating of individualized patient care plans.

  • Conduct regular case conferences and clinical team meetings to ensure continuity of care and effective communication.

  • Monitor clinical documentation for accuracy, timeliness, and adherence to quality standards.

  • Participate in quality improvement initiatives, audits, and performance reviews.

  • Assist with hiring, orientation, and ongoing education of clinical staff.

  • Maintain appropriate staffing levels and manage scheduling to ensure adequate coverage.

  • Serve as a clinical resource and mentor for the care team.

  • Collaborate with referral sources, physicians, and community partners to coordinate services and promote agency growth.

  • Respond to patient concerns or incidents with professionalism and follow-up as needed.

Qualifications:

  • Active Registered Nurse NY

  • Bachelor's degree in Nursing (BSN) preferred

  • Minimum of 3–5 years of clinical nursing experience in home health, with at least 2 years in a supervisory or leadership role.

  • Strong understanding of home health regulations, including OASIS, Medicare/Medicaid guidelines, and electronic health records (EHR).

  • Excellent leadership, communication, and organizational skills.

  • Ability to multitask, problem-solve, and work in a fast-paced environment.
