Overview

We are seeking a dedicated and compassionate Occupational Therapist to join our healthcare team. The ideal candidate will provide comprehensive therapy services to patients across various settings, including inpatient, schools, and skilled nursing facilities. This role involves developing personalized care plans, assisting patients with activities of daily living (ADLs), and supporting recovery and functional independence. The Occupational Therapist will work closely with multidisciplinary teams to deliver high-quality patient care tailored to individual needs, including those with disabilities, autism, geriatrics, pediatrics, and post-acute conditions.

Requirements

  • Valid NYS Occupational Therapist License
  • Experience working in various settings such as inpatient hospitals, outpatient clinics, skilled nursing facilities, home health, or schools is preferred
  • Knowledge of physiology, anatomy, medical terminology, and case management principles
  • Familiarity with assisting patients with ADLs and discharge planning processes
  • Experience working with populations including, adults (geriatrics), individuals with autism or disabilities, and those requiring post-acute or acute care services
  • Ability to develop individualized care plans based on patient assessments
  • Strong communication skills for working effectively with patients, families, and multidisciplinary teams
  • Ability to adapt therapy approaches for different environments
  • Knowledge of speech therapy integration is a plus but not required

KEY RESPONSIBILITIES include but not limited to:

  • Assess patients’ physical, cognitive, and emotional needs to develop effective treatment plans
  • Assist patients with activities of daily living (ADLs) such as dressing, grooming, and mobility
  • Implement therapeutic interventions aimed at improving motor skills, coordination, and functional independence
  • Conduct discharge planning to ensure smooth transition from hospital or facility to home or community settings
  • Collaborate with physicians, case managers, speech therapists, and other healthcare professionals to coordinate care
  • Work with diverse populations including pediatrics (early intervention and school-based), geriatrics, individuals with autism or disabilities, and those in acute or post-acute care settings
  • Maintain accurate documentation of patient progress and update care plans regularly
  • Provide education and training to patients and caregivers on therapeutic exercises and adaptive techniques
  • Participate in team meetings and contribute to quality improvement initiatives within the facility
  • Provides patient care in a non-judgmental, non-discriminatory manner that considers cultural diversity and age appropriateness so that autonomy, rights, and dignity are preserved.
