Job Title: Case Manager - Health and Recovery Plan (HARP)
Location: Nassau or Suffolk County, NY
Schedule: Monday-Friday, 9:00 AM - 5:00 PM
On-site: 2 days/week in office; field-based role
Caseload: 35 individuals



Position Overview:

We are seeking a dedicated Case Manager to support adults with significant behavioral health needs under the New York State Health Home Model. This role focuses on the integration of physical health, mental health, and substance use services, with access to Home and Community-Based Services (HCBS) for eligible individuals. Case Managers play a key role in coordinating care and connecting individuals to essential supports and services in the community.



Key Responsibilities:

  • Conduct outreach, intake, assessments, and reassessments.

  • Manage a caseload by coordinating physical, behavioral health, and HCBS services.

  • Develop and maintain individualized care and service plans.

  • Collaborate with managed care organizations, service providers, and community-based resources.

  • Ensure individuals maintain access to benefits and entitlements.

  • Monitor participation in treatment, medical follow-ups, and service utilization.

  • Assist with appointment scheduling and follow-up care post-discharge.

  • Perform home visits and field-based outreach as needed.

  • Maintain accurate documentation using electronic health record systems.

  • Support crisis intervention and connect individuals to appropriate services when necessary.



Qualifications:

Required:

  • Bachelor's degree in Social Work, Psychology, Counseling, Rehabilitation, Sociology, or a related human services field.

  • At least 2 years of experience working with individuals with serious mental illness, developmental disabilities, homelessness, or similar populations.

  • Ability and willingness to perform regular community fieldwork.

  • Strong organizational, communication, and time management skills.

  • Proficiency with electronic documentation systems and digital communication tools.

Preferred:

  • Master's degree in a related field (with at least 1 year of relevant experience).

  • Familiarity with Medicaid, Social Security, and other public benefit systems.

  • Bilingual (Spanish or other languages) a plus.



Additional Information:

  • Full-time position, Monday-Friday, 9:00 AM to 5:00 PM

  • Hybrid schedule: office-based, remote, and fieldwork

  • Interview follow-up expected within 24-48 hours
