1115 Waiver Enhanced Navigator – Catholic Charities of Fulton and Montgomery Counties
The 1115 Waiver Enhanced Navigator plays a crucial role in supporting individuals as they navigate available services, helping them access life-changing resources and empowering them on their journey toward wellness and stability.
In this role, you will:
- Conduct screenings with Medicaid-eligible individuals using the approved screening tools — ensuring accuracy, compassion, and proper documentation.
- Enter and maintain timely, accurate data in the Unite Us platform, meeting all 1115 Waiver requirements.
- Educate clients, referral partners, and community members about waiver eligibility and available services.
- Participate in required trainings, including Unite Us, trauma-informed care, HIPAA, and compliance.
- Collaborate with case managers and internal teams to deliver coordinated, client-centered care.
Position Details

Salary: $21.00/hour

Schedule: Monday–Friday, 35 hours a week
Qualifications

Associate’s Degree in Human Services or a related field

1–3 years’ experience in mental health, substance use, or human services
