SUMMARY

The Children’s Health Home Manager is responsible for the overall daily operations of our Children’s Health Home Program and staff. The Manager directs the development, coordination, integration, and delivery of Health Home care coordination services by staff that meaningfully engage partners in care. Additionally, the Manager ensures that all required documentation is correctly completed by staff in a timely manner and completed in compliance with Health Home standards.


RESPONSIBILITIES & DUTIES:

The duties and responsibilities of the position are to be carried out in a manner that is consistent with the mission, values, and operating principles of Jewish Family Services.

• Supervise the day-to-day operations of the program including management of caseload across staff.

• Supervise and provide leadership to staff, assuring the delivery of high-quality patient care, including adherence to professional standards of care, licensing, and regulatory requirements.

• Effectively communicate job expectations and follow-up by planning, monitoring, and appraising job results.

• Ensure files, case notes, and documentation are all in compliance with program requirements.

• Facilitate weekly staff meetings and provide on-going staff trainings.

• Ensure that staff complete the CANS assessment for clients and assist with this task as needed.

• Ensure that care coordinators complete the monthly High, Medium, Low Assessment for monthly health home billing and assist staff in this task as needed.

• Monitor staff performance of core service delivery, including monthly core service delivery percentage.

• Assist staff with delivering services to clients as needed, including providing caseload coverage during times of vacancy.

• Work with the Director of Care Coordination to monitor the effectiveness of Outreach and Enrollment Specialist; offer suggestions on increasing enrollments and ensure department leadership is aware of caseload/staffing ratios.

• Complete all required chart audits in compliance with the lead health home, along with on-going chart reviews and work in collaboration with the Director of Care Coordination on other quality assurance activities.

• Conduct hiring and on-boarding activities, including the training and mentoring of new staff.

• Work in collaboration with the Director of Care Coordination and other agency managers to both increase internal referrals to Health Home and to ensure Health Home clients are utilizing any other needed agency services.

• Help identify new funding opportunities and care coordination programs to support adult populations in need.

• Attend departmental, partner, and agency meetings as required.


QUALIFICATIONS:

Education and Experience:

• Bachelor’s degree in human services with two years of relevant experience or Master degree in human services with one year of relevant experience.

• Applicable background and experience relevant to Health Homes.

• 1-3 years program management or supervisory experience preferred.

Knowledge, Skills & Abilities:

• Oversee the Children Health Home staff, providing leadership and mentoring of staff.

• Work collaboratively with the care team to develop and maintain a person-centered care planning process.

• Network with other community agencies and organizations for the purpose of reaching clients in need and coordinating services.

• Collaborate across JFS programs to ensure integration of services and maximize use of client resources.

• Ability to operate at a high level of independence and responsibility

• Knowledge of the local service provider networks, particularly related to health homes.

• Ability to interpret and respond to program and funder guidelines.

• Exhibited ability to effectively work within an inclusive and culturally and linguistically diverse environment.

• Strong internet research and computer skills, especially with Outlook, Word, Excel, PowerPoint, and web-based health information systems.

WORKING CONDITIONS

• Will work in the office and in the community; able to travel outside the office to various sites to attend meetings and provide support services.

• Must have access to a reliable vehicle, possess a valid, clean driver’s license and be sufficiently self-insured with liability insurance in the amount of $100/$300k.

• Flexible hours including occasional evenings and/or weekends.

PHYSICAL REQUIREMENTS

• Physical activities and efforts required working in an office environment.

• Visual acuity sufficient to maintain system of files and reports containing computer-generated and handwritten documents.

• Auditory acuity sufficient to communicate with staff, clients, and others by phone and in person.

• Mobility sufficient to conduct regular duties within a normal office environment and community.

COMPENSATION & BENEFITS

  • Competitive salary range of $58,500 - $65,000, commensurate with experience and qualifications.
  • Potential for quarterly bonuses based on productivity.

The above pay range is a good faith estimate for the position at the time of posting. Final compensation may vary based on factors including, but not limited to, background, knowledge, skills, and abilities.
