Job Summary
YWCA NENY Rapid Rehousing program serves individuals with varying levels of mental health diagnoses, women recovering from addiction, those recently released from incarceration, individuals living in extreme poverty, chronically homeless individuals and other marginalized populations. Reporting to the Rapid Rehousing Coordinator, the Case Manager is responsible for a caseload of approximately 30 participants/households who are homeless or at risk of homelessness due to domestic violence.

FLSA Category: Non-exempt
Compensation: $19 - $21.50 Hourly.
Responsibilities:
- Conduct intake screening with client once referral received to determine housing needs.
- Assess client’s physical and mental needs and refer for assistance as needed.
- Assist with household budgeting and lease management for apartments. Refer individuals for financial counseling as needed.
- Provide ongoing case management as needed after family has found housing, to help maintain safety, security and mediate landlord tenant dispute as needed. Case Management may be located in client’s home (within the capital region) or YWCA NENY office.
- Encourage and provide guidance to clients with services such as social services, SSI/SSDI, finding employment, continuing education, obtaining childcare etc.
- Participate in apartment inspections with clients.
- Maintain files and case notes (empower database data entry) for individuals/families in the program.
- Participate in conferences, staff meetings, trainings.
- Other duties as assigned.
Required Skills/Abilities:
- Knowledge and experience managing program budget and expenses.
- Knowledge and experience with NYS tenant and/or landlord rights.
- Knowledge of Housing and Urban Development (HUD) requirements and standards.
Qualifications:
- 2-4 year’s experience working with individuals needing assistance with low-income housing preferred.
- Associates degree in Human Services or related field preferred.
- Clean NYS Driver’s license and ability to use own vehicle for work related travel.
- Proficiency with Microsoft Office, Internet use, and database.
Physical Requirements
- Prolonged periods of sitting at a desk.
- Ability to use computer and necessary agency software.
- Able to lift up to 25 pounds without team assistance.
- Occasional bending, standing, reaching, stair climbing.
