Compass Healthcare Consulting & Placement is conducting a search for a Coordination Manager for a Licensed Home Care Services Agency (LHCSA) located in Brooklyn, NY. Qualified candidates will have extensive Manager or Supervisor level Coordination experience within a Licensed Home Health Care Agency, will have the ability to mentor and coach coordination staff, provide support and guidance to all levels of staff with a professional and approachable Managerial style, will be experienced with HHAeXchange or similar Home Care Scheduling System. Coordination Manager - Home Care - LHCSA
The Coordination Manager in the Licensed Home Care Services Agency oversees delivery of in-home care services for HHA/PCA, NHTD and Private Duty Services, ensuring high-quality patient care and compliance with healthcare standards. This role is responsible for managing a team of Home Care Coordinators, following established processes and recommending changes to enhance efficiency and quality, addressing escalations with patients, their families, and healthcare providers, and promoting continuous improvement in service delivery. Works collaboratively with Executive Leadership, Directors, Managers, Coordinators, and Scheduling Staff to bring optimal quality and care to clients.
Responsibilities:
- Supervise daily operations of the Coordination Department to ensure timely and accurate caregiver scheduling in accordance with client needs and regulatory standards and patient rights (e.g., under 10 NYCRR 766.2).
- Support and guide a team of Coordinators, providing day-to-day oversight, performance feedback, training and professional development to ensure service excellence and team effectiveness
- Oversee case assignments, monitor schedule changes, address urgent coverage needs, and coordinate crisis interventions to minimize service disruptions and ensure patient safety.
- Ensure all assigned Caregivers are fully compliant with DOH and agency requirements prior to placement, including verifying certifications, and ensuring supervisory visits occur
- Serve as the first point of escalation for scheduling conflicts, Caregiver concerns, client service issues, or quality complaints, resolving matters promptly and professionally while documenting resolutions for compliance
- Collaborate with Intake, Compliance, HR, and Field Operations, and external partners (e.g., MLTC plans, hospitals, etc.) to support seamless transitions from on-boarding to active case assignments, care plan updates, and follow-up services.
- Review and audit schedule entries and caregiver records in the agency’s scheduling platform (e.g., HHAeXchange) to ensure data accuracy and completeness
- Monitor team productivity, adherence to internal processes, and key performance indicators; identifying opportunities for improvement, implementing workflow enhancements, and lead quality improvement initiatives
- Prepare and present department updates, reports, and metrics to management, including staffing coverage metrics and service performance indicators
- Foster a collaborative and supportive work environment, promoting open communication and accountability within the team

Qualifications:
- Associates Degree, preferred
- 5+ years of experience in Home Care Coordination, with at least 3+ years in a Supervisory or Team Lead role
- Strong knowledge of Licensed Home Care Services Agency (LHCSA) and New York State Department of Health (DOH) regulations, Caregiver compliance requirements (HHA/PCA training, certification, and supervision), and Medicaid/MLTC eligibility and billing processes.
- Proficiency in scheduling software such as HHAeXchange or other home care management platforms
- Excellent organizational and time management skills, with the ability to prioritize and manage multiple demands in a fast-paced environment
- Strong leadership and team management abilities, with a proven track record of mentoring and developing staff
- Effective problem-solving and conflict resolution skills, particularly in managing Caregiver callouts, schedule changes, and coverage issues
- Clear and professional communication skills, both written and verbal, with the ability to interact effectively with Caregivers, clients, internal departments and external stakeholders (e.g., MLTC plans).
- Bilingual (English+Spanish) is a plus
