Pay Range: $65,000.00 - $95,000.00 per year

We are seeking a Full-Time Discharge Planner to join our team!

Position Overview

As the Discharge Planner, you will play a critical role in helping residents and their families navigate the transition from our facility to their next level of care or home. You will coordinate with interdisciplinary teams, ensure all discharge requirements are met, and facilitate appropriate aftercare services. Your attention to detail, communication skills, and compassion will be essential in supporting successful and safe discharges.

Key Responsibilities

Discharge Coordination

  • Plan and coordinate all aspects of resident discharges in collaboration with the clinical and social services teams.
  • Ensure discharge planning begins at admission and is updated throughout the resident’s stay.
  • Maintain accurate and up-to-date discharge documentation.

Communication & Collaboration

  • Serve as the primary liaison between residents, families, medical providers, and outside agencies.
  • Educate residents and families on post-discharge services, including home care, medical equipment, transportation, and community resources.
  • Coordinate with hospitals, physicians, and other care providers as needed.

Care Transition Support

  • Arrange follow-up care and services to ensure continuity and reduce the risk of hospital readmission.
  • Assist residents with identifying support systems and provide referrals for necessary services.
  • Document discharge plans and related communications in accordance with facility policies.

Qualifications

  • Bachelor’s Degree in Social Work, Human Services, Nursing, or a related field preferred, not required.
  • Experience in discharge planning or case management within a healthcare, rehabilitation, or long-term care setting preferred.
  • Strong understanding of healthcare systems, post-acute care services, and community resources.
  • Excellent communication, organizational, and problem-solving skills.
  • Compassionate, patient-centered approach.
  • Proficient in electronic medical records and Microsoft Office Suite.
