Position Summary: Under the supervision of a clinical pharmacist or appointed preceptor, develop and maintain a comprehensive program for high-quality and coordinated medication management through delivery of medication-related care to help achieve measurable outcomes and improve patients’ quality of life. Additionally, the resident will complete teaching activities in both the didactic and experiential course.
Major Duties & Responsibilities:
Under supervision of an appointed preceptor include, but are not limited to:
- Perform patient assessments to ensure the appropriateness of medication therapy and enhance patients’ understanding of their medications.
- Monitor the effects of medication therapy.
- Provide medication-related information to patients and clinicians.
- Communicate with practitioners in a timely manner regarding new medications, new medication warnings and medication recalls.
- Recommend laboratory tests necessary for monitoring outcomes of therapy.
- Interpret data related to medication safety and effectiveness.
- Document the care provided in patients’ records and update medication list as appropriate.
- Identify barriers to medication adherence and work with patients to improve adherence.
- Communicate with payers to resolve issues that may impede access to specific medications.
- Identify cost containment opportunities.
- Assess for opportunities to refer the patient to other members of the clinical care team including, social workers, case managers, care navigators, and diabetes or nutrition educators.
- Perform medication reconciliation, patient counseling, transitional care services and patient and health care provider education.
- Support practice transformation initiatives as part of a multi-disciplinary patient care team.
- Collaborate with payors on value-based care programs as appropriate
- Teach in small or large classroom settings.
- Precept pharmacy students on experiential learning experiences
- Assist with development of continuing education for practitioners on issues pertaining to pharmacotherapy.
- Completion of scholarly activities with the goal of presenting at a national conference
- Other duties may be assigned to meet business needs.

- Proficiency in the use of the Microsoft Office suite, especially Excel, Outlook and Word.
- Familiarity with electronic medical records and ambulatory care environment.
- Strong teaching aptitude and desire to work with students.
- A clear understanding of experiential learning.
Education and Experience:
- PharmD Degree, completion of post-graduate year 1 residency.
- Eligibility for licensure in New York State.
- Immunization certification desirable.
Physical Demands:
While performing the duties of this job, the employee is regularly required to sit; use hands to finger, handle, or feel; and talk or hear. The employee frequently is required to reach with hands and arms. The employee is occasionally required to stand; walk; climb or balance; and stoop, kneel or crouch. The employee must occasionally lift and/or move up to 10 pounds. Specific vision abilities required by this job include close vision, and the ability to adjust focus. Work Environment: While performing the duties of this job, the employee is not substantially exposed to adverse environmental conditions.
Additional Information: This position is based in Albany, NY. The duties and responsibilities of this particular role require the need for consistent, in-person collaboration and/or interaction with our community, requiring all work to be performed full-time on-site. Employee will need to travel to various PGY-2 site locations. Standard work hours are from 8:30 AM – 4:30 PM EST, Monday through Friday. Additional details pertaining to this position will be discussed during the interview process.
