We are seeking a compassionate and experienced Nurse Care Manager to join our client’s team in supporting members of a Managed Long-Term Care (MLTC) program. This role focuses on coordinating care for individuals with chronic illnesses, disabilities, or other long-term health needs. The Nurse Care Manager will work closely with members, their families, and interdisciplinary teams to assess needs, create care plans, and ensure access to the resources required for optimal health outcomes and quality of life.
Key Responsibilities
- Conduct in-home, telephonic, or facility-based assessments to evaluate members' physical, functional, and psychosocial needs.
- Identify healthcare, long-term care, and social service requirements.
- Develop and implement person-centered care plans in alignment with MLTC program requirements.
- Coordinate services, including home care, medical appointments, therapies, and community resources.
- Collaborate with members, families, providers, and MLTC staff to ensure continuity and appropriateness of care.
- Serve as a member advocate, ensuring their care needs are met while respecting individual preferences.
- Educate members and caregivers on disease management, self-care, and available resources.
- Ensure compliance with MLTC guidelines, including timely completion of reassessments and care plan updates.
- Maintain accurate documentation of assessments, care plans, and communications in the electronic health record (EHR) system.
- Monitor member outcomes, identify changes in condition, and update care plans as needed.
- Participate in quality improvement initiatives to enhance service delivery and member satisfaction.
Requirements:
- Bachelor of Science in Nursing (BSN) required.
- Current and unrestricted Registered Nurse (RN) license in NY.
- Certification in Case Management (CCM) or equivalent preferred.
- Minimum of 3 years of clinical nursing experience, with at least 1 year in case management, care coordination, or MLTC.
- Knowledge of MLTC programs, home health, and community-based services is required.
- Strong understanding of managed care principles and Medicaid/MLTC regulations.
- Excellent assessment, problem-solving, and organizational skills.
- Ability to work independently and as part of an interdisciplinary team.
- Must speak Chinese
