RN Case Manager Needed in Eastern Orange County Territory (Cornwall, Highlands, Monroe, Woodbury, Blooming Gove, Chester)

Focused On Dignity, Comfort, And Support, The Organization Offers Comprehensive Services To Patients And Families Both At Home And In Dedicated Care Settings.
Salary and Benefits – RN Case Manager – Eastern Orange County Territory
- $87,000 - $105,000 (Based on Experience
- PTO Up to 136.5 Hours/Year In The First Four Years
- Increases To 174 Hours/Year After Five Years
- 6 Paid Holidays
- Full Medical, Dental, and Vision Insurance
- Mileage Reimbursement: Paid From Patient To Patient
Key Responsibilities – RN Case Manager
- Monday To Friday: 8:30 AM – 4:30 PM
- Evening and Weekend Call Rotation: Approximately Every 5-6 Weeks
- Provide Skilled Nursing Care and Symptom Management in Patient Homes
- Develop and Implement Individualized Care Plans With the Hospice Team
- Educate Patients and Families on Care, Medications, and Disease Management
- Coordinate Care with Physicians and Interdisciplinary Team Members
- Document Patient Care Accurately with EMR – Healthwise
Requirements:
- Active New York RN License
- 1 Year of Hospice or Palliative Care Experience -Preferred
- 2 Years of in–Home Patient Care Experience
- Familiarity with EMR System with Healthwise
