Job DescriptionDescription:

Care Management is a service model designed to ensure comprehensive communication and coordination among all caregivers involved in a resident's care. A designated "care manager" takes the lead, overseeing and providing access to the necessary services to address the resident's needs, preventing hospitalizations, maintaining health, and ensuring stability.

The Care Manager is responsible for providing and coordinating services for their assigned caseload. This involves guiding program enrollees and their legal guardians through the healthcare system, addressing access issues, building relationships with service providers, and monitoring interventions and outcomes.

Requirements:

Bilingual in Spanish & English A Must

Ø Obtain required Care Management enrollment consent from individuals or legal guardians.

Ø Conduct initial and ongoing needs assessments (Child and Adolescent Needs and Strengths; CANS) to determine the most appropriate level of care management.

Ø Manage the patient’s Individualized Plan of Care, coordinating services, supporting treatment adherence, and monitoring various needs, including prevention, wellness, medical mental health, care transitions, and social and community services.

Ø Meet client contact requirements, including face-to-face visits based on acuity levels.

Ø Fulfill Care Management documentation requirements using designated tools like Foothold Care Management HER (FCM) and Health Commerce Systems (HCS) such as UAS-NY and MAPP applications

Ø Serve as an advocate for clients within the agency and with external service providers.

Ø Promote wellness and prevention by connecting enrollees with resources based on individual needs and preferences including but not limited to ameliorating gaps in care and managing medical/dental appointments.

Ø Educate children/caregivers on the care of chronic conditions, immunization, screening, and preventive interventions.

Ø Assist clients in obtaining and maintaining public benefits essential for healthcare services.

Ø Effectively communicate and share information with individuals, families, and other caregivers, considering language, literacy, and cultural preferences.

Ø Facilitate interdisciplinary Team meetings to aid in care plan development, overall wellness and health promotion.

Ø Identify available community-based resources, complete referrals, promote engagement and follow-up of implemented services.

Ø Participate in discharge planning for hospital admissions, ensuring discharge recommendation adherence.

Ø Satisfy required ongoing staff development training as recommended by Agency, DOH and Children’s Health Home

Ø Follow up on all treatment care per Medicaid EPSDT guidelines.

Ø Perform other duties as assigned by the Care Management Agency.

MINIMUM QUALIFICATIONS:

Ø Bachelor of Arts or Science with two years’ experience (Complex/Intense Acuity members)

PREFERRED EXPERIENCE:

Ø Valid driver’s license with a good driving record. (Plan to attain within 60 days of hire)

Ø Efficient knowledge of/familiarity with CANS 2.0 assessment and Care Management software (FCM/UAS-NY/MAPP)

PREFERRED SKILLS/REQUIREMENTS:

Ø Proficiency in computer software packages (MS Word, Excel, Outlook, Google office, MS-Teams)

Ø Excellent oral and written communication skills.

Ø Problem-solving solid capability and capacity to meet multiple deadlines under pressure.

Ø Highly organized and detail-oriented.

Ø Relevant expertise and experience in serving children and families in child welfare, developmental disabilities, mental health, healthcare, and/or other systems, as well as those receiving preventive services.

Ø Care Managers serving high acuity enrollees should demonstrate knowledge and understanding through additional experience, education, or training.

Ø Proven self-management abilities, meeting deadlines, ensuring compliance with agency policies, and overseeing complete and timely maintenance of agency records.

Ø Care Managers for children with medical fragility must have extensive experience coordinating their care.

Ø Experience providing service coordination, information, linkages, and referrals for community-based services.

Ø Ability to travel to community-based agency offices, communities, and homes within the assigned catchment area.

BACKGROUND VERIFICATION and CLEARANCE:

Ø Credential Verification.

Ø Medical clearance, including TB screening.

Ø Section 424-A of the New York Social Services Law requires an inquiry into a candidate's child abuse and maltreatment report.

Ø State Central Registry Clearance.

Ø Staff Exclusion List (SEL).

Ø Justice Center Background Check

REQUIRED TRAINING/CERTIFICATION:

Ø Child and Adolescent Needs and Strengths (CANS) certified (will train upon hire if n not certified)

Ø Care Management 101 (3 volume series)

Ø Mandated Reporter

Ø Lasallian Culture of Care (LCC).

Ø All other mandatory training and certifications as required by Agency, DOH,HHSC, OMH State, and/or Federal guidelines.

PHYSICAL REQUIREMENT:

Ø Lifting requirement of 50 lbs.

Ø Ability to meet physical requirements of the position during standing, walking, sitting, bending, stooping, squatting, kneeling, and climbing.

Salary $25.00-27.00 /hr
