TSINY is seeking a Full-Time Adult Home Care Coordination Supervisor for their BHCC-M program.
Salary: $55,000; $30.21/Hr., Full-Time
Position Summary
The Adult Home Care Coordination Supervisor (Supervisor) provides clinical and administrative supervision to Adult Home Care Coordinators who are providing intensive care management services to individuals identified as Adult Home (AH) class members under the Stipulation and Order of Settlement, United States v. State of New York. The Supervisor oversees the care coordination, care planning, and other activities of the Adult Home Care Coordinator. The Supervisor also ensures that Adult Home Care Coordinators address unmet needs that impact on the transition from the adult home into the community.
Essential Functions
- Supervise all Adult Home Care Coordinators.
- Assist in the interviewing process of candidates applying for Adult Home Care Coordinator positions.
- Assist in the training of Adult Home Care Coordinators.
- Evaluate the class member’s interest in transitioning from the adult home to the community.
- Educate the class member on the benefits of transitioning to the community.
- Team to provide support consistent with the care plan and to foster ongoing skill building and independence.
- Re-assess the class member’s needs five months after transition to determine if the class member still requires Adult Home Plus case management services. Graduate the class member from these services if they are no longer needed. Maintain the class member if these services are still needed.
- Continue with weekly contacts until the class member graduates from Adult Home Plus services.
- Document all Health Home Core Services and other services provided.
- Services as Adult Home Care Coordinator in the absence of assigned staff or staffing vacancies.
- Be able to sit or stand as needed, with or without reasonable accommodation.
- Be able to travel independently by public transportation.
- May require walking, primarily on a level surface, for short periods throughout the day, with or without reasonable accommodation.
- Be able to reach above shoulder heights, below the waist or lift as required to file documents or store materials throughout the workday, with or without reasonable accommodation.
- During a declared disaster, assume and adhere to assigned Job Action role(s) consistent with Disaster Preparedness Plan.
- Perform other related duties as required.
Experience/Education/Skills/Abilities
- Licensed level healthcare professional (e.g., RN, licensed clinician, psychologist) with prior experience in a behavioral health clinic or care management supervisory role; or Master’s level professional with three (3) years prior experience supervising clinicians and/or care managers who are providing direct care services to individuals with Serious Mental Illness (SMI) or Substance Use Disorder (SUD).
