Early Intervention Ongoing Service Coordinator
Hours: M-F 8-4
12-month position
Salaried position with benefits
Salary range $32-37,000 (probationary training period)

Education
Applicant must possess a Bachelor’s Degree in Health, Education or a related field
Job Responsibilities
Provide services as an ongoing service coordinator (OSC) in Early Intervention, including:
Initial contact with families
Locating providers to service families
Participating in Individual Family Service Plan (IFSP) meetings and transitioning to the Committee on Special Education (CPSE) meetings
Serve as liaison between families and the Nassau County Department of Health Early Intervention program
Candidate must possess:
Ability to be patient and empathetic with families
Ability to work collaboratively with colleagues, families and the Nassau County Department of Health
Excellent verbal and written communication skills
Good organizational skills
Bilingual a plus
