Cayuga Counseling Services, Inc. a private non-for-profit health and human services agency has a Full time Care Coordinator position in our Onondaga office. The Onondaga Care Coordinator position is a joint partnership between Cayuga Counseling Services and the Onondaga County Probation Department to operate an enhanced pre-trial release program. The program seeks to address the needs of defendants housed in the Onondaga County Justice Center who suffer with a serious mental health illness and possibly a co-occurring substance abuse disorder with the goal to have the defendant released and remain in the community as their case proceeds.

The Care Coordinator will perform the following duties:

  • Receive referrals from the Town, Village, Onondaga County and Syracuse Criminal Court Judges.
  • Ensure court attendance and compliance of program conditions with clients and send notifications to the court for noncompliance.
  • Upon release of defendant, identify and assist with establishing additional community related linkages to services.
  • Communicate with service providers and the assigned Probation Officer to ensure client participation and attendance
  • Document in a timely manner all contact with service providers and clients in the Onondaga County computer database
  • Attend specialty court twice per week to report client compliance and any other changes to the judge
  • Track and compile program data
  • Assist with Pre-Trial Release case manager duties as assigned.

Qualified candidates will have:

  • Bachelor's degree in social work or a related human services field
  • 3+ years' experience with intensive case management/care coordination
  • Have the ability to develop and foster relationships.
  • Excellent verbal and written communication skills.
  • Prior experience with the criminal justice population is a plus.

The Agency offers:

  • 3 Weeks PTO in your first year, 4 weeks in your second year.
