We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will provide high-quality patient care in nursing homes. This role requires strong clinical skills, the ability to work collaboratively with healthcare professionals, and a commitment to delivering exceptional care to patients of all ages.

Duties

  • Administer medications and treatments as prescribed by physicians.
  • Perform patient assessments and monitor vital signs.
  • Assist with daily living activities and provide personal care to patients.
  • Document patient care accurately in electronic health records (EHR) systems.
  • Collaborate with registered nurses and other healthcare team members to develop and implement patient care plans.
  • Provide education to patients and their families regarding health management and disease prevention.
  • Participate in triage and emergency response as needed.
  • Maintain compliance with HIPAA regulations and infection control protocols.
  • Support patients with chronic conditions, including Alzheimer’s care, dementia care, and pain management.

Skills
Candidates should possess a combination of the following skills:

  • Proficiency in using EMR systems such as Epic, Cerner, Athenahealth, or eClinicalWorks.
  • Experience in critical care, acute care, or emergency medicine settings.
  • Knowledge of medical terminology, ICD-9/ICD-10 coding, and medical documentation practices.
  • Familiarity with procedures such as IV insertion, catheterization, phlebotomy, and specimen collection/processing.
  • Strong understanding of geriatrics, senior care, hospice care, and home health practices.
  • Ability to perform physical examinations and assess patient needs effectively.
  • Experience working with individuals with developmental disabilities or special needs.
  • Skills in case management, discharge planning, and health coaching.
  • Competence in performing basic life support (BLS) and airway management techniques.
