Job DescriptionCare Navigator (Remote - Contract-to-Hire)

Pay: $20-$23/hour
Start Date: December 1st
Contract Length: 5 months, with potential for hire
Location: Fully Remote (U.S.-based only)
Schedule: Full-time | Monday-Friday, 9:00 AM - 6:00 PM EST (40 hours per week)
Training: Virtual, 1-2 weeks
Equipment: Not provided initially; may be provided long-term



Overview

Medix Healthcare Staffing is seeking a Care Navigator to support a statewide initiative focused on improving access to healthcare and social support services for Medicaid members. The Care Navigator plays a key role in helping individuals understand, access, and enroll in available benefits and programs, including services offered through the 1115 Waiver.

This is a high-volume outbound call position-the majority of the day will be spent reaching out to members, completing initial assessments, and connecting them with appropriate health and social service resources.



Key Responsibilities

  • Conduct a high volume of outbound calls to Medicaid members to discuss benefits, assess needs, and determine eligibility for enhanced services.

  • Complete structured assessments of members' health status and living situations to identify Health-Related Social Needs (HRSNs) such as housing, food, and transportation.

  • Educate members about programs available under the 1115 Waiver and assist with enrollment or connection to services.

  • Obtain and document member consent and attestation as required.

  • Record all outreach attempts, interactions, and progress notes accurately and in compliance with program standards.

  • Communicate with internal teams, community partners, and external organizations to ensure referrals are accepted and completed.

  • Meet outreach and engagement goals through persistence, organization, and proactive communication.



Qualifications

  • Strong communication and customer service skills with the ability to build trust and rapport over the phone.

  • Experience working with Medicaid populations or public health programs preferred.

  • Bilingual (Spanish/English) preferred.

  • Detail-oriented, organized, and able to meet outreach goals in a fast-paced, remote environment.

  • Comfortable with outbound calling and engaging members who may be difficult to reach.



Preferred Experience

  • Familiarity with NYS Medicaid and the 1115 Waiver.

  • Knowledge of case management or care coordination principles (note: this role focuses on navigating care, not managing care).

  • Familiarity with regions such as the Finger Lakes, Hudson Valley, Bronx, or Capital District.

  • Experience working in a start-up or program implementation environment.
