Job Description
Endocrine Associate of West Village PC is a physician-led multispecialty practice, focused on the continuous support, growth, and accessibility of high-quality patient-focused care.
Candidate must be a CDCES, certified through NCBDE (National Certification Board for Diabetes Educators. Alternately an active NYS Licensed RD and CD (both required), or active NYS RN license, or active NYS Nurse Practitioner license, with 2-5 years’ experience in diabetes education and management.
Candidate must have pump certification for devices currently on the market (Medtronic, Tandem, Omnipod, etc) and knowledge of Type I, Type II, and gestational diabetes.
As part of the education team, you work closely with Endocrinology providers and actively interact to promote a team approach.
Job Duties
- Provide one-on-one consultation for patients on diabetic devices while individualizing education to patients, their families, and/or significant others based on their needs assessment.
- Provide diabetes self-management education by educating patients, who have pre-diabetes or diabetes, about lifestyle changes, prevention strategies, and disease management practices.
- Responsibilities include but are not limited to, working with a diabetic patient to help them understand their needs; medication dosages and diabetes monitoring practices such as testing blood sugar levels, use of CGM, insulin pumps, etc.
- Responsible for continuous follow-up with patients to access their ability in managing their diabetes; collecting patient data and sharing progress reports with other medical professionals.
- Have the ability to teach diabetes classes.
- Must provide appropriate documentation within EMR.
- Work with diabetes education team for a comprehensive diabetes device program that is tailored to each patient.
- Able to follow-up on requests from other providers, assistants, ancillary staff and patients promptly via phone, email or patient portal.
- Attend monthly meetings with CDCES staff and medical director.
Job Requirements
- 2 – 5+ years of relevant, professional work experience in outpatient counseling with Type 1/Type 2/Gestational Diabetes population(s).
- Knowledge of diabetes management and various diets that correlate with other comorbidity factors such as CHF, hyperlipidemia, kidney disease, HTN, GI, etc.
- Knowledge of subcutaneous insulin pump therapy and assessment of continuous glucose monitoring and sensors.
- Support diabetic patients to help them attain optimal body weight.
- Excellent written and verbal communication skills.
- Strong teaching, counseling, and motivational skills.
- Establish and maintain effective working relationships with both staff and patients.
- Competency in Microsoft Office (Word, Excel, PowerPoint, etc).
- Must be able to finish notes in a timely manner.
- Document all information discussed, written materials and training provided.
- Evaluate patient understanding, and comprehension of the education provided is a must.
- Ability to read, speak and write clearly.
- Be able to work in a fast-paced environment.
- Able to work up to five days a week: Day shift 9am-5pm.
- Must meet vaccination requirements.
