We offer hands-on help to address chronic homelessness, mental health, substance abuse, disability, and systematic poverty by offering innovative solutions and proactive services to the most vulnerable in Schenectady County.
The Intensive Case Manager will provide outreach and crisis management services to individuals in emergency shelters (ES). The ICM will respond to the immediate housing need. In addition, the ICM will work with the Agency Social Work Department and Medical Care staff to respond to the social determinants of the individual's health needs. The ICM will work directly with Schenectady County DSS to ensure all individuals placed in ES, particularly hotels/motels, are expeditiously exited out of the temporary housing and into stable housing. In addition, this position will set up appointments, attend appointments as necessary, and provide direct intensive case management as needed.
Primary Responsibilities:
- Engage in daily communication with Schenectady County Department of Social Services.
- Hold weekly Home Connections orientation meetings for new referrals.
- Complete the DSS “Daily Snapshot and Discharge” form and email it to DSS and supervisor.
- Provide Outreach: On-site presence at emergency shelters, specifically hotels/motels.
- Process intake and assessments of clients identifying as homeless and/or at risk of becoming homeless.
- Collaborate with other Agency Case Management staff for housing placement - utilize providers for possible subsidy funding.
- Collaborate with the Clinical Department for engagement with mental and medical health needs.
- Provide assessment needs plan to supervisor.
- Maintain on-going evaluation and revision of treatment/service plan as needed.
- Establish and maintain satisfactory working relationship with community providers.
- Maintain confidentiality, appropriate boundaries, and utilize required reporting formats (including HMIS) and complete documents and reports as required.
- Client data entry: HMIS Home Connections ES ~ when in emergency shelter
- Client data entry: HMIS Home Connections CM ~ when placed in housing and providing 12 months of case management
- Data entry: HMIS Coordinated Entry
- Provide client specific documentation to Case Management aiding in compliance for all grant and governmental regulations.
- NYS Shelter Management monitoring and auditing: be available at time of audit and present case files.
- Maintain comprehensive physical and electronic client files.
- Hold strong belief that crisis can be prevented through early intervention and collaboratively working with others.
- Adhere to the agency’s policies and procedures as well as the personnel manual.
Qualifications:
- Bachelors Degree in Human Services with five years full-time, paid experience or combination of education and experience
- Five years of experience providing services to the homeless or related human services necessary
- Solid organizational skills and strong work ethic needed
- Must be a self- starter and able to successfully work independently.
- Strong written and verbal communication skills essential
- Computer literacy needed
- Knowledge of agencies, programs, and services in the community.
- Knowledge of eligibility criteria and application processes for mainstream/entitlement programs helpful
- Valid NYS Driver’s License for at least one year.
- No DWI or DWAI within the last three years and the ability to be covered by the Agency’s vehicle insurance.
- Ability to pass the Agency’s road test.
- Must be able to provide own transportation.
- Must successfully complete a background investigation
- Engage community resources as needed.
- Will demonstrate resourcefulness, work independently, and take initiative
