Key Position Responsibilities – Independent Living Skills Trainer (ILST)

Provide individuals with disabilities with information and guidance on disability-related topics and independent living services.

Make appropriate referrals to external agencies for services not offered by Guided Steps NY, Inc.

Conduct intake assessments to collaboratively determine the consumer's needs and the scope of independent living services required.

Develop and complete an Independent Living Plan (ILP) for each consumer, based on their personal goals and preferences; obtain a signed waiver if the consumer opts out of ILP participation.

Deliver individualized skills training aligned with the goals outlined in the ILP.

Maintain accurate and up-to-date consumer service records, including documentation of follow-up on referrals.

Ensure timely submission of all required paperwork and data entry in accordance with agency and departmental protocols.

Provide peer counseling when appropriate and/or refer consumers to qualified peer support professionals.

Offer regular updates on consumers within your assigned caseload.

Engage in education and advocacy efforts on behalf of consumers and the broader community.

Actively participate in community events and outreach activities.

Perform additional duties as assigned by supervisors or leadership.
