Part & Full Time Positions Available

Key Position Responsibilities – Independent Living Skills Trainer (ILST)

  • Provide individuals with disabilities with information and guidance on disability-related topics and independent living services.

  • Make appropriate referrals to external agencies for services not offered by Guided Steps NY, Inc.

  • Conduct intake assessments to collaboratively determine the consumer's needs and the scope of independent living services required.

  • Develop and complete an Independent Living Plan (ILP) for each consumer, based on their personal goals and preferences; obtain a signed waiver if the consumer opts out of ILP participation.

  • Deliver individualized skills training aligned with the goals outlined in the ILP.

  • Maintain accurate and up-to-date consumer service records, including documentation of follow-up on referrals.

  • Ensure timely submission of all required paperwork and data entry in accordance with agency and departmental protocols.

  • Provide peer counseling when appropriate and/or refer consumers to qualified peer support professionals.

  • Offer regular updates on consumers within your assigned caseload.

  • Engage in education and advocacy efforts on behalf of consumers and the broader community.

  • Actively participate in community events and outreach activities.

  • Perform additional duties as assigned by supervisors or leadership.
