RESPONSIBILITIES

  • Provide trauma-informed services to clients and their families as they exit shelter and settle into permanent housing. Work with clients will include intake, engagement, assessment, service plan development, referrals, advocacy, counseling, and follow-up.
  • Manage a caseload of approximately 40-50 families – three to six months being the average length of an open case.
  • Conduct 50 face-to-face client contacts each month.
  • Collaborate with BronxWorks Family Shelter social services staff to identify and recruit potential program participants.
  • Aid clients in establishing and maintaining open public assistance cases, positive relationships with property managers, and pathways to problem-solving issues that may arise in their new home or community.
  • Coordinate client services with other social services, mental health, medical, legal, employment, education, and childcare providers, as needed.
  • Document all interactions with, or on behalf of, clients. Monitor and document client progress toward service plan goals.
  • Maintain familiarity with community resources.
  • Refer clients and families to services or resources relevant to their goals.
  • Contribute ideas for enhancing and expanding services provided to clients and families through the Transform Program.
  • Report to, and meet with, supervisor on a bi-weekly basis.
  • Perform additional duties as assigned.

QUALIFICATIONS

  • Bachelor’s degree in social work or another related field of study.
  • One-year of case management or relevant work experience preferred.
  • Engaging personality and detailed orientated skills are essential.
  • Strong oral and written communication, time management and organizational skills are necessary.
  • Proficiency in Microsoft Office suite and other standard business technology is required.

PHYSICAL REQUIREMENTS

  • Ability to use a computer for prolonged periods
  • Ability to occasionally lift and/or move up to 10 pounds
  • Ability to stand, walk, or sit for long periods of time
  • Ability to climb five flights of stairs, if required to conduct home visits and/or fieldwork
  • Ability to bend and retrieve objects and/or documents
  • Ability to travel in the boroughs of New York City and its adjacent counties via public transportation
  • Ability to travel for a minimum of 1 mile within a reasonable amount of time and in all types of weather, including inclement weather, if required to conduct home visits and/or field work
  • ‘TB Test required with the first 120 days of employment’ for Shelters and Homeless Outreach programs
