Job description:

<li>Setting: Skilled Nursing Facility, Sub-acute and long-term care patients.</li>
<li>Schedule: Minimum of (2) days on site per week. Telehealth support is available on-site for virtual interactions.</li>
<li>Must be licensed in NY </li>
<li>Experienced in SNF preferred, not required. </li>
