Nurse Practitioner – Primary Care
Location: New York, NY
Schedule: Full-Time | 9:00 AM – 6:00 PM
Salary: $155,000 – $165,000/year

A mission-driven healthcare organization is hiring a Nurse Practitioner (NP) to provide comprehensive adult care in a community-based setting. This role emphasizes patient-centered care, collaboration, and chronic disease management.

About the Role:
You’ll work alongside primary care physicians to assess, diagnose, and treat adult patients, including those with complex medical needs. The role blends clinical independence with team-based coordination and a strong focus on improving care outcomes.

Key Responsibilities:

  • Conduct annual wellness exams, routine follow-ups, and acute care visits

  • Manage chronic illnesses with tailored treatment plans

  • Complete post-hospital discharge visits and medication reconciliations

  • Supervise and ensure care plan delivery for primary care participants

  • Provide in-person and telehealth care as appropriate

  • Ensure timely, accurate documentation in the EHR (Athena)

  • Collaborate with multidisciplinary care teams and refer to specialists when needed

  • Represent the Medical Director when required

  • Participate in quality improvement initiatives

Qualifications:

  • Graduate of an accredited NP program; board-certified (AANP or ANCC)

  • Active NYS licenses as both NP and RN

  • DEA certification required

  • 2+ years of clinical nursing experience in primary care or related setting

  • 1+ year working with adult or elderly populations (or willingness to train)

  • Strong communication, documentation, and care coordination skills

  • Bilingual preferred

Additional Requirements:

  • Medicaid and Medicare provider IDs in good standing

  • Immunizations up to date; medically cleared for communicable diseases

  • Able to meet physical demands of ambulatory care and travel between sites
