Position: Clinical Dietitian
Shift: Days (40 hours/week) – Rotating weekend coverage
Overview:
We are seeking a Clinical Dietitian who will be responsible for ensuring the overall nutritional well-being of our patients and residents. The Dietitian will collaborate closely with the medical staff and interdisciplinary team to develop and implement individualized care plans that promote optimal health outcomes for our patients, residents, and their families.
Responsibilities:
- Conduct comprehensive nutrition assessments and identify residents at risk for malnutrition.
- Perform Nutrition Focused Physical Exams (NFPE) to determine the degree of malnutrition.
- Develop and implement individualized diet plans, monitoring the effectiveness of nutrition interventions.
- Provide nutrition education and counseling to patients/residents, families, and facility staff.
- Collaborate with healthcare providers to ensure nutrition care aligns with overall treatment goals.
Requirements:
- Bachelor of Science in Human Nutrition and Dietetics required.
- Registered Dietitian (RD) or Registration-Eligible required.
- Certified Dietitian/Nutritionist (CDN) in New York State preferred.
- Minimum 1 year of experience in acute care or long-term care preferred.
- Strong communication, interpersonal, and teamwork skills.
- Proficiency in PointClickCare, Meditech, and Microsoft Office Suite (Word, Excel, Outlook).
- Demonstrated commitment to excellent customer service.
