Job Title: Patient Navigator
Location: Brooklyn, NY 11215
Contract Duration: 13 Weeks
Schedule: Monday–Friday | 10:00 AM – 6:00 PM
Pay Rate: Up to $19/hour
Position Overview
"Navitas Healthcare, LLC" is seeking a compassionate and detail-oriented Patient Navigator to support oncology patients within a hospital-based Cancer Service Line. This role focuses on care coordination, patient advocacy, and ensuring a seamless healthcare experience throughout the patient’s treatment journey.
Key Responsibilities

Assist oncology patients in navigating the healthcare system and accessing appropriate services

Provide patient coordination and basic case management support

Communicate effectively with patients, families, and multidisciplinary care teams

Deliver culturally sensitive, patient-centered support

Maintain accurate and timely documentation using basic computer applications
Required Qualifications

High School Diploma or GED

Excellent verbal and written communication skills

Prior experience working directly with patients in a healthcare setting

Basic computer proficiency (Microsoft Word, Excel)

Strong customer service skills with cultural awareness and sensitivity
Preferred Qualifications

Bachelor’s Degree from an accredited college or university

Previous case management experience

Bilingual in Spanish
