Wound Care Nurse Practitioner (Skilled Nursing Facilities)

Job DescriptionFirst Care Providers is a trusted leader in delivering wound care services to skilled nursing and long-term care facilities. We are committed to improving patient outcomes and enhancing quality of life through evidence-based care and collaboration.
We are looking for a Nurse Practitioner with 1+ years of wound care experience to manage wound care in our contracted skilled nursing and long-term care facilities. Full or Part time opportunities are available. Flexible hours. No weekend call.
Key Responsibilities:
- Provide advanced wound care, assessments, and treatments at skilled nursing facilities
- Develop individualized care plans and monitor healing progress
- Collaborate with facility staff, physicians, and care teams
- Educate staff and families on wound prevention and care best practices
- Complete thorough documentation in our medical records system
Qualifications:
- Active Nurse Practitioner license (NY) (REQUIRED)
- Wound care experience (REQUIRED)
- Wound care certification (e.g., WCC, CWCN, CWON) (PREFERRED)
- Strong clinical, communication, and organizational skills
- Ability and willingness to travel between facilities
