Job Description

Job DescriptionRole Overview:

Assist consumers in accessing community services (medical, psychological, substance use, vocational, food, clothing, etc.) through referrals, advocacy, and follow-up.

Schedule:

  • 9a-5p, Tuesday to Saturday (Full Time/Contract with Opportunity for Extension)
  • Hourly Rate: $24/hr.

Key Responsibilities:

  • Manage caseloads and develop individualized support plans.
  • Provide supportive counseling, problem-solving, and crisis intervention.
  • Accompany consumers to appointments and assist with daily living needs.
  • Advocate for consumers and encourage self-advocacy.
  • Coordinate community-based services and activities for social and skill development.
  • Document services and maintain relationships with therapeutic support services.
  • Assist with moves and on-call crisis support as needed.

Requirements:

  • Bachelor's Degree
  • 2 years' experience in behavioral health.
  • Proficient in Microsoft Office and general computer programs.

Preferred Qualifications:

  • Experience with mental illness, substance use, or homelessness.
  • Background in case management, community outreach, or supportive housing.
  • Bilingual English/Spanish preferred.
