Job DescriptionRole Overview:
Assist consumers in accessing community services (medical, psychological, substance use, vocational, food, clothing, etc.) through referrals, advocacy, and follow-up.
Schedule:
- 9a-5p, Tuesday to Saturday (Full Time/Contract with Opportunity for Extension)
- Hourly Rate: $24/hr.
Key Responsibilities:
- Manage caseloads and develop individualized support plans.
- Provide supportive counseling, problem-solving, and crisis intervention.
- Accompany consumers to appointments and assist with daily living needs.
- Advocate for consumers and encourage self-advocacy.
- Coordinate community-based services and activities for social and skill development.
- Document services and maintain relationships with therapeutic support services.
- Assist with moves and on-call crisis support as needed.
Requirements:
- Bachelor's Degree
- 2 years' experience in behavioral health.
- Proficient in Microsoft Office and general computer programs.
Preferred Qualifications:
- Experience with mental illness, substance use, or homelessness.
- Background in case management, community outreach, or supportive housing.
- Bilingual English/Spanish preferred.
