Job DescriptionAre you a compassionate and skilled Licensed Practical Nurse (LPN) ready to make a meaningful impact in the lives of individuals recovering from mental health and substance abuse challenges? We are looking for a dedicated LPN to join our supportive team at a transitional home in Queens. This is a wonderful opportunity to contribute to a nurturing environment while providing essential care for those on their recovery journey.
What You’ll Do: Monitor adult and geriatric patients in a residential setting, ensuring their safety and well-being.
Take and record vital signs, closely tracking health data to support patient care.
Conduct basic assessments, communicating findings to the care team to ensure comprehensive treatment.
Collaborate with RNs and Med Tech staff, providing support and sharing information for the best patient outcomes.
Assist in maintaining a safe and therapeutic environment, helping foster a calm, supportive space for patients.
Document patient progress accurately using NextGen EMR for seamless communication and record-keeping.
What We’re Looking For: Active NY LPN license
Prior experience in mental health or substance abuse care is preferred
Compassionate and team-oriented, with a passion for working in a collaborative environment
Strong communication skills, enabling effective coordination with the care team

Stable 12-Hour Shifts: Enjoy a consistent schedule with shifts from 6:00 AM – 6:00 PM or 6:00 PM – 6:00 AM.
