Shift: Hourly 8 Hour Minimum

Location: Primarily Dutchess County and Westchester County; additional assignments across New York as needed

Job Responsibilities:

• Provide nursing care in accordance with care plans under RN supervision
  
  
• Administer prescribed medications and treatments
  
  
• Monitor patient health and report changes to the supervising RN/physician
  
  
• Assist with patient education and support families in care practices
  
  
• Document patient status and care provided accurately and timely
  
  
• Work collaboratively with RNs, PCAs, HHAs, and the care team
  
  

Qualifications:

• Current NYS Licensed Practical Nurse (LPN) license in good standing
  
  
• Meets NYS health requirements (recent physical, immunizations)
  
  
• CPR certification (required)
  
  
• Minimum 1 year of nursing experience (home care preferred)
  
  
• Positive and compassionate attitude toward patients and families
  
  
• Drivers License or reliable transportation is required
  
  

License/Certification:

• LPN (Required)
