Nurse Practitioner – House Calls (Driver Provided)
Location: Bronx, NY
Position Type: Full-Time | Permanent
Salary Range: $130,000 – $200,000 (based on experience)
Position Overview
We are seeking a compassionate and skilled Nurse Practitioner (NP) to provide high-quality, patient-centered care through home visits in the Bronx, NY area. This role focuses on serving patients with chronic illnesses, mobility challenges, or limited access to traditional healthcare settings.
The NP will deliver holistic, in-home primary care while managing complex medical conditions, emphasizing patient education, continuity of care, and collaborative practice.
Key Responsibilities
- Conduct comprehensive in-home assessments, including physical exams and medication reconciliation
- Diagnose and manage acute and chronic medical conditions
- Develop and implement individualized treatment plans
- Prescribe medications and order diagnostic testing as appropriate
- Coordinate care with physicians, specialists, and interdisciplinary teams
- Provide education to patients and families on disease management and preventive care
- Document patient encounters accurately and thoroughly in EMR/EHR systems
- Perform follow-up visits to monitor progress and adjust care plans
- Address urgent medical needs during home visits
- Participate in community outreach initiatives
Qualifications
- Master’s Degree in a Nurse Practitioner program (Family or Adult Geriatrics preferred)
- Active New York State Nurse Practitioner license
- National Board Certification (AANP or ANCC required)
- Bilingual Spanish is a plus
- EMR/EHR experience preferred

Work Environment
- Primarily home-based patient care (varied living conditions)
- Travel within assigned geographic area (driver provided)
