<p>Responsibilities Include:</p>

<li>Conduct intake interviews</li>
<li>Guide individuals and families in need through critical social service systems</li>
<li>Create personalized service plans</li>
<li>Find and refer clients to community resources from housing to healthcare</li>
<li>Handle documentation including Medicaid status and program eligibility</li>
<li>Maintain detailed client files and ensure proper follow-up through ongoing assessment</li>
<li>Maintain client files</li>
<li>Strong interpersonal skills</li>
<li>Sympathetic, empathetic</li>
<li>Good communication</li>
<li>Reliable, hard worker</li>
<li>Pay $23-$25</li>
<li>BA Degree</li>
<li>Bilingual a plus</li>
