Case Manager
Responsibilities Include:
- Conduct intake interviews
- Guide individuals and families in need through critical social service systems
- Create personalized service plans
- Find and refer clients to community resources from housing to healthcare
- Handle documentation including Medicaid status and program eligibility
- Maintain detailed client files and ensure proper follow-up through ongoing assessment
- Maintain client files
- Strong interpersonal skills
- Sympathetic, empathetic
- Good communication
- Reliable, hard worker
- Pay $23-$25
- BA Degree
- Bilingual a plus
