NPORT is currently seeking a dedicated and compassionate Physical Therapist (PT) to join our team in Middletown, NY. This role offers the opportunity to provide high-quality rehabilitative care in both SNF and home care environments. We are looking for clinicians who are passionate about improving patients' mobility, restoring function, and enhancing quality of life.

Responsibilities:

• Perform evaluations and develop individualized treatment plans
• Provide physical therapy services in SNF and CHHA settings
• Accurately document patient progress per company and regulatory standards
• Work collaboratively with an interdisciplinary care team
• Educate patients and families on therapy plans and home exercise programs

Requirements:

• Active Physical Therapist (PT) license in New York State
• Prior SNF or home care experience a plus (not required)
• Strong clinical and communication skills
• Ability to work independently and travel locally for home care visits
