Job Summary:
The Field Nurse Supervisor is responsible for overseeing the delivery of skilled nursing care in patients’ homes, ensuring clinical excellence, regulatory compliance, and patient satisfaction. This role involves supervising field nurses, conducting home visits, coordinating care plans, and serving as a key liaison between clinical teams, patients, and families. The ideal candidate is a licensed RN with strong leadership, communication, and assessment skills and a passion for quality home health care.
Key Responsibilities:

Supervision & Leadership:

Provide clinical oversight and supervision of field RNs, LPNs, and aides.

Ensure nursing staff are delivering care in accordance with physician orders, care plans, and agency policies.

Conduct field evaluations and competency assessments.

Patient Care Oversight:

Perform skilled nursing visits as needed, including admissions, recertifications, supervisory visits, and discharges.

Monitor and manage clinical progress and patient outcomes.

Communicate effectively with patients, families, and physicians regarding care plans and changes in condition.

Compliance & Documentation:

Review and approve OASIS documentation, clinical notes, and visit records for accuracy and compliance.

Ensure all care provided meets state, federal, and accreditation standards (e.g., Medicare, Joint Commission).

Participate in quality assurance and performance improvement (QAPI) activities.

Training & Development:

Assist in the orientation, training, and ongoing development of field staff.

Educate nurses on best practices, new procedures, and regulatory updates.

Collaboration:

Work closely with the Director of Nursing, case managers, and schedulers to ensure optimal staffing and continuity of care.
