$28 an hour
Overseeing: Sullivan, Orange, and Ulster Counties
Position Summary: The Care Manager Supervisor, Health Homes, is responsible for ensuring that Care Managers provide adequate care management. In collaboration with staff, effectively develop, implement, supervise, and administer policies, procedures, and personnel to accomplish objectives of the program. Work within all New York State Department of Health requirements. Maintain collaborative, working relationships with the Health Home and Community Partners.
Responsibilities:
- Complete required reports and documentation in a timely manner meeting all standards of content and professional presentation.
- Ensure that Health Home and Agency standards of practice as well as State, Federal, and Accreditation standards are fully met, which includes managing cases as needed.
- Continuously maintain and enhance positive relationships with Health Home as well as other community schools, service providers and service recipients.
- Ensure the standards of a high performing team member are role modeled on a daily basis and that supervisees are accountable for performing to the same standard.
- Ensure continuity of personnel fully competent in achieving objectives through effective selection, development, training, evaluation and motivation of key supervisees.
Requirements
Education:
- Bachelors degree required
- A waiver for an associates degree plus extensive employment history can be pursued on a case-by-case basis
Experience:
- 2 years of experience in with youth or case management required
- Driver's License and ability to maintain insurance throughout employment required
