RESPONSIBILITIES:
- Health Insurance Enrollment: Enroll clients into appropriate health insurance programs and provide comprehensive post-enrollment assistance.
- Client Education and Support: Provide clients with information, education, advice, and navigational services on how to effectively utilize their health insurance.
- Intake and Screening Services: Conduct intake, screening, and enrollment services for clients eligible for the NON-MAGI Medicaid (Aged, Blind, Disabled) FE-ABD program.
- Address Consumer Health Needs: Develop solutions to address consumer health coverage needs, including dental coverage, billing issues, and other areas of under-insurance.
- Patient Education and Referrals: Offer patient education and make referrals for additional services as needed to support client health and wellness.
- Workshop Facilitation: Provide workshops on health-related topics to educate and empower clients.
- Maintain Case Records: Keep thorough case records of services provided, advice given, and referrals made, using both internal and external databases.
- Advocacy and Follow-Up: Advocate for clients and provide follow-up services to ensure their continued progress and well-being.
- Compliance: Ensure strict adherence to HIPAA compliance standards, maintaining the confidentiality and security of all client information.
- Funder Compliance: Comply with all funder requirements, including reporting, documentation, and service delivery standards.
- Community Engagement: Participate in community outreach initiatives.
- Case Conferences: Hold regular case conferences with clients to review and adjust service plans as needed.
- Program Reporting: Complete and submit program reports on a daily basis or as required.
- Supervisory Meetings: Report to and meet with your supervisor on a monthly basis to review caseloads and discuss any issues or progress.
- Additional Duties: Perform other responsibilities identified by the supervisor to support the overall mission and effectiveness of the agency.
Qualifications:
- High school diploma, bachelor’s degree preferred.
- Bilingual both verbal and written preferred.
- Proficiency in modern business communications including personal computers, electronic mail, voicemail, facsimile, and copier equipment.
- One year experience preferred.
Essential duties and Responsibilities:
- Provide direct services to clients, including intake, engagement, assessment, service plan development, referral, advocacy, exit summaries, and follow-up.
- Manage a case load.
- Monitor and document client progress toward service plan goals.
- Document all interaction with, or on behalf of, clients.
- Maintain familiarity with program resources available on- and off-site.
- Conduct regular case conferences with clients and service providers as required.
- Complete data entry on a daily basis.
- Report to, and meet with, supervisor on a monthly basis.
- Conduct outreach, as required.
- Perform additional duties as assigned.
Physical Requirements:
- Ability to use a computer for prolonged periods.
- Ability to occasionally lift and/or move up to 10 pounds.
- Ability to stand, walk, or sit for long periods of time.
- Ability to bend and retrieve objects and/or documents.
- Ability to travel in the boroughs of New York City and its adjacent counties via public transportation.
- Ability to travel for a minimum of 1 mile within a reasonable amount of time and in all types of weather, including inclement weather, if required to conduct home visits and/or field work.
